血清胃蛋白酶原Ⅰ/Ⅱ、胃泌素-17水平在早期胃癌筛查中的应用价值

摘　　要：目的 探讨血清胃蛋白酶原Ⅰ/Ⅱ(PGⅠ/PGⅡ)、胃泌素-17(G-17)水平在早期胃癌筛查中的应用价值。方法 选取胃癌患者120例作为胃癌组、胃良性病变患者50例作为胃良性病变组,并选取同时期体检的正常人群30例作为对照组,采用胶乳增强免疫比浊法检测血清PGⅠ、PGⅡ,化学发光法检测G-17水平,并计算PGⅠ/PGⅡ,二元Logistic回归分析胃良性病变、胃癌组结果,受试者工作特征曲线(ROC)分析PGⅠ/PGⅡ、G-17对胃癌的预测价值。结果三组患者年龄、CEA、CA19-9、CA72-4、PGⅠ、PGⅡ、PGⅠ/PGⅡ、G-17比较,P<0.05;三组患者性别比较,P>0.05。Logistic回归分析显示:CEA、CA19-9、CA72-4、PGⅡ、G-17均为胃癌危险因素(P<0.05),PGⅠ、PGⅠ/PGⅡ为胃癌保护因素(P<0.05)。ROC曲线显示,与单一检测指标比较,PGⅠ/PGⅡ、G-17联合预测胃癌的AUC为0.778(95%CI:0.707~0.849),灵敏度为0.633,特异度为0.820,P<0.001。结论 PGⅠ/PGⅡ、G-17血清学检测在早期胃癌筛查中具有一定的应用价值,联合检测对胃癌的预测价值较单一检测指标更高。Objective To investigate the application value of serum pepsinogenⅠ/Ⅱand gastrin-17 levels in early gastric cancer screening.Methods A total of 120 patients with gastric cancer were selected as the gastric cancer group,50 patients with benign gastric lesions were selected as the benign gastric lesions group,and 30 normal people were selected as the control group at the same time.The serum PGⅠand PGⅡlevels were detected by lateenhanced immunoturbidimetric method,and the level of G-17 was detected by chemiluminescence method,and PGⅠ/PGⅡwas calculated.Binary Logistic regression was used to analyze the results of gastric benign lesions and gastric cancer group.Receiver operating characteristic curve(ROC)was used to analyze the predictive value of PGⅠ/PGⅡand G-17 serological detection for gastric cancer.Results Age,CEA,CA19-9,CA72-4,PGⅠ,PGⅡ,PGⅠ/PGⅡand G-17 were compared between the three groups(P<0.05),and gender was compared between the 3 groups(P>0.05).Logistic regression analysis showed that CEA,CA19-9,CA72-4,PGⅡand G-17 were risk factors for gastric cancer(P<0.05),and PGⅠand PGⅠ/PGⅡwere protective factors for gastric cancer(P<0.05).ROC curve showed that the AUC of PGⅠ/PGⅡcombined with G-17 for predicting gastric cancer was 0.778(95%CI:0.707-0.849),the sensitivity was 0.633,and the specificity was 0.820,P<0.001.Conclusion PGⅠ/PGⅡand G-17 serological detection have certain application value in early gastric cancer screening,and combined detection has higher predictive value for gastric cancer than single detection index.
