摘 要:目的 探讨膀胱容量测量仪(Bladder volume measuring instrument)监测膀胱尿量在宫颈癌调强放疗(intensity modulated radiotherapy, IMRT)中的临床意义。方法 选择44例宫颈癌患者,随机分为研究组和对照组,各22例。所有患者均接受调强放疗,研究组采用膀胱容量测量仪监测膀胱尿量,对照组通过患者主观感觉确定膀胱尿量。比较2组患者在三维方向的靶区位移、均匀性指数(HI)、适形度指数(CI)以及膀胱、小肠、直肠受照射体积百分比,并比较2组放疗期间出现的肠道不良反应。结果 研究组在左右、腹背和头脚方向的位移误差均显著小于对照组(P<0.05)。研究组HI显著低于对照组(P<0.05),但2组CI差异无统计学意义(P>0.05)。研究组膀胱、小肠、直肠的受照射体积百分比显著低于对照组(P<0.05)。2组腹泻发生率的差异无统计学意义(P>0.05),但对照组患者出现腹泻的时间早于研究组。结论 在宫颈癌调强放疗(IMRT)前,采用膀胱容量测量仪对膀胱尿量进行监测来维持膀胱充盈程度的一致性,可有效减少靶区位移误差,增加靶区勾画的精准性,保证靶区精准照射剂量,同时有效地保护了膀胱、小肠、直肠,减轻了放疗副反应,值得临床推广应用。Objective To study bladder volume measurement clinical application in cervical carcinoma intensity modulated radiotherapy(IMRT).Methods Forty-four cervical carcinoma patients were enrolled to receive IMRT and patients were assigned randomly to the research group and the control group.The research group were assigned to monitor bladder volume with bladder volume measuring instrument.The control group were assigned to estimate bladder volume by subjective impression.Three-dimensional target displacement,homogeneity index(HI),conformity Index(CI),bladder small intestine and large intestine irradiated volume,and intestinal adverse reactions were compared between the 2 groups.Results Target displacement errors of left-right direction,abdominal and dorsal direction,and head and foot direction were significantly lower in research group compared to control group(P<0.05).HI of the research group was significantly lower than that of the control group(P<0.05).CI of research group was not significantly different than that of the control group(P>0.05).The percentage of bladder,small intestine and large intestine irradiated volume was significantly lower than that of the control group(P<0.05).Diarrhea incident time was early in the control group but diarrhea incidence rate was not significantly different between the 2 groups(P>0.05).Conclusion Bladder volume measurement clinical application in monitoring bladder volume could help to achieve consistent bladder filling degree,reduced target displacement,improved target accuracy,improved target irradiation dose,and improved protection of bladder small intestine and large intestine.Bladder volume measurement clinical application could improve cervical carcinoma intensity modulated radiotherapy efficacy and deserve further clinical applitation.
