摘　　要：目的 分析脑膜瘤患者Ki67、FASN、HER-2表达水平及其对脑膜瘤切除术后复发的预测价值。方法 收集98例脑膜瘤患者临床资料。依照患者术后复发情况分为复发组(22例)和非复发组(76例)。比较两组一般资料;比较复发组和非复发组Ki67、FASN、HER-2表达水平;分析Ki67、FASN、HER-2表达水平对脑膜瘤切除术后复发的预测价值。结果 两组性别、年龄、肿瘤位置、组织类型、肿瘤重量、切除方式比较差异无统计学意义(P>0.05),两组Ki67、FASN、HER-2表达差异显著(t=6.658,P=0.000,χ2=3.351,P=0.008,χ2=3.626,P=0.000)。将脑膜瘤切除术后复发=1,非复发=0作为因变量,Ki67、FASN、HER-2表达作为自变量,进行逻辑回归及ROC曲线分析,结果显示,Ki67、FASN、HER-2对脑膜瘤切除术后复发的预测价值良好,AUC分别为0.865、0.720、0.745,灵敏度分别为0.818、0.545、0.682,特异度分别为0.829、0.816、0.711。三者联合对脑膜瘤切除术后复发的预测效能相对单一指标较高,AUC最大为0.894,灵敏度为0.773,特异度为0.895。Ki67的预测阈值为4.050%。结论 脑膜瘤切除术后复发患者Ki67、FASN、HER-2表达水平均呈异常高表达,且三者对于脑膜瘤切除术后复发有良好的预测价值,可能作为临床上脑膜瘤切除术后复发的评价因子。Objective To analyze the expression levels of Ki67,FASN and HER-2 in patients with meningioma and their predictive value for recurrence after meningioma resection.Methods The clinical data of 98 patients with meningioma were collected.Patients were divided into recurrence group(22 cases)and non-recurrence group(76 cases).Compare the 2 sets of general data;The expression levels of Ki67,FASN and HER-2 were compared between the relapsed group and the non-relapsed group.To analyze the value of Ki67,FASN and HER-2 expression levels in predicting the recurrence of meningioma after resection.Results There were no significant differences in gender,age,tumor location,tissue type,tumor weight and resection method between the 2 groups(P>0.05).There were significant differences in the expressions of Ki67,FASN and HER-2 between the 2 groups(t=6.658,P=0.000,χ2=3.351,P=0.008,χ2=3.626.P=0.000).Recurrence=1 and non-recurrence=0 after meningioma resection were taken as dependent variables,and the expressions of Ki67,FASN and HER-2 were taken as independent variables.Logistic regression and ROC curve analysis were performed,and the results showed that Ki67,FASN and HER-2 had good predictive value for recurrence after meningioma resection.AUC was 0.865,0.720 and 0.745,sensitivity was 0.818,0.545 and 0.682,and specificity was 0.829,0.816 and 0.711,respectively.Compared with a single index,the combined efficacy of the three indexes was higher in predicting the recurrence of meningioma after resection.The maximum AUC was 0.894,the sensitivity was 0.773,and the specificity was 0.895.The prediction threshold of Ki67 is 4.050%.Conclusion The expression levels of Ki67,FASN and HER-2 are abnormally high in patients with recurrence after meningioma resection,and the 3 have good predictive value for recurrence after meningioma resection,and may be used as clinical evaluation factors for recurrence after meningioma resection.
