血清AMH、CA125、β-HCG联合检测对卵巢癌的早期诊断价值

摘　　要：目的 探讨血清AMH、CA125、β-HCG联合检测在卵巢癌早期诊断中的应用价值。方法 选取卵巢肿瘤患者86例,包括恶性40例(恶性组)、良性46例(良性组),另选取同期体检的健康女性40例作为对照组。比较三组之间以及不同阶段卵巢癌患者之间的血清AMH、CA125、β-HCG水平,并进一步分析三项指标单独及联合检测在卵巢癌诊断中的价值。结果 与良性组和对照组相比,恶性组患者血清AMH水平显著降低(P<0.05),而CA125、β-HCG水平显著升高(P<0.05);与Ⅰ~Ⅱ期卵巢癌患者相比,Ⅲ~Ⅳ期卵巢癌患者AMH水平显著降低(P<0.05),CA125、β-HCG水平显著升高(P<0.05);同时,与单独检测相比,血清AMH、CA125、β-HCG的联合检测灵敏度明显更高,差异具有统计学意义(P<0.05),联合检测的特异性也明显更高,但差异无统计学意义(P>0.05)。结论 卵巢癌患者血清AMH呈低表达,CA125、β-HCG呈高表达,且其水平变化与疾病进展密切相关,联合检测血清中AMH、CA125和β-HCG可以有效提高卵巢癌早期诊断的准确性,具有较高的临床应用价值。Objective To investigate the value of serum AMH,CA125,β-HCG combined test in the early diagnosis of ovarian cancer.Methods Eighty-six patients with ovarian tumors were selected,including malignant 40 cases(malignant group)and benign 46 cases(benign group),and another 40 healthy women with the same period of physical examination were selected as the control group.The serum AMH,CA125 andβ-HCG levels were compared among the 3 groups and among patients with different stages of ovarian cancer,and the value of the 3 indexes in the diagnosis of ovarian cancer individually and in combination was further analyzed.Results Compared with the benign and control groups,the serum AMH level of patients in the malignant group was significantly lower(P<0.05),while the levels of CA125 andβ-HCG were significantly higher(P<0.05);compared with the patients with stageⅠ~Ⅱovarian cancer,the AMH level of patients with stageⅢ~Ⅳovarian cancer was significantly lower(P<0.05),and the levels of CA125 andβ-HCG were significantly higher(P<0.05);Meanwhile,compared with separate detection,the combined detection of serum AMH,CA125 andβ-HCG was significantly more sensitive,and the difference was statistically significant(P<0.05),and the specificity of combined detection was also significantly higher,but the difference was not statistically significant(P>0.05).Conclusion Ovarian cancer patients have low expression of serum AMH and high expression of CA125 andβ-HCG,and the changes of their levels are closely related to disease progression.Combined detection of serum AMH,CA125 andβ-HCG can effectively improve the accuracy of early diagnosis of ovarian cancer,and it has high clinical application value.
