外周血T淋巴细胞亚群表达水平对多发性骨髓瘤患儿预后的预测价值

摘　　要：目的探究外周血T淋巴细胞亚群表达水平对多发性骨髓瘤(MM)患儿预后的预测价值。方法收集90例MM患儿临床资料进行回顾性分析。根据患儿预后情况将其分为良好组(n=68例)和不良组(n=22例)。比较两组患儿的一般资料、CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、NK细胞的表达情况,并分析外周血T淋巴细胞亚群相关指标表达水平对MM患儿预后的评估价值。结果两组患儿年龄、性别、M蛋白、血肌酐比较差异不显著(P>0.05)。不良组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK细胞均显著低于良好组(t=3.276,P=0.002、t=6.635,P=0.000、t=2.242,P=0.028、t=6.271,P=0.000),CD8^(+)高于良好组,差异有统计学意义(t=3.603,P=0.001)。外周血T淋巴细胞亚群相关指标CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、NK细胞的表达水平在MM患儿预后的评估中有良好的应用价值,AUC分别为0.701、0.880、0.711、0.621、0.860、0.959,上述指标联合应用对MM患儿预后的预测效能最高,AUC最大,为0.959,灵敏度为0.935,特异度为0.921。CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、NK细胞的预测阈值分别为57.252%、34.255%、42.241%、0.847、22.144%。结论外周血T淋巴细胞亚群表达水平对MM患儿预后有良好的预测价值,可将外周血T淋巴细胞亚群检测作为临床上MM患儿预后的评估方法。Objective To investigate the prognostic value of T lymphocyte subsets expression in peripheral blood in children with multiple myeloma.Methods The clinical data of 90 children with MM were retrospectively analyzed.According to the prognosis,the children were divided into good group(n=68 cases)and bad group(n=22 cases).The general data,expressions of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)and NK cells in the 2 groups were compared,and the value of expression levels of peripheral blood T lymphocyte subsets in evaluating the prognosis of MM children was analyzed.Results There were no significant differences in age,sex,M protein and serum creatinine between the 2 groups(P>0.05).CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and NK cells in the bad group were significantly lower than those in the good group(t=3.276,P=0.002,t=6.635,P=0.000,t=2.242,P=0.028,t=6.271,P=0.000),and CD8^(+)were higher than those in the good group.The difference was statistically significant(t=3.603,P=0.001).The expression levels of peripheral blood T lymphocyte subsets related indicators CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)and NK cells have good application value in the prognosis assessment of children with MM,with AUC values of 0.701,0.880,0.711,0.621,0.860,0.959,respectively.The combined application of the above indexes had the highest prognostic efficacy,the highest AUC(0.959),the highest sensitivity(0.935)and the highest specificity(0.921)in MM children.The predicted thresholds of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)and NK cells were 57.252%,34.255%,42.241%,0.847 and 22.144%,respectively.Conclusion The expression level of T lymphocyte subsets in peripheral blood can predict the prognosis of MM children,and the detection of T lymphocyte subsets in peripheral blood can be used as a prognostic factor in MM children.
