床边B超引导下经皮穿刺扩张气管切开术在神经重症患者中的临床应用

摘　　要：目的:探讨床边B超引导下经皮穿刺扩张气管切开术在神经重症患者中的临床应用价值。方法:选择2023年9月—2024年8月怀集县人民医院神经外科、重症监护病房收治的神经重症患者70例,按照随机数字表法分为对照组和试验组,每组35例,比较两组患者的一次穿刺成功率、手术操作时间、术中出血量、相关并发症。结果:试验组患者一次穿刺成功率高于对照组(P<0.05);试验组手术操作时间、术中出血量指标低于对照组(P<0.05);试验组围术期并发症发生率低于对照组(x^(2)=3.968,P=0.046);试验组远期并发症率低于对照组(x^(2)=6.437,P=0.011)。结论:床边B超引导下经皮穿刺扩张气管切开术在治疗神经重症患者中的应用效果较好,可有效提高穿刺成功率,缩短手术操作时间,减少术中出血量,避免造成组织损伤,减少并发症发生率,具有很高优势,可在未来临床治疗中加以推广和实施。Objective:To investigate the clinical application value of percutaneous dilatation tracheotomy guided by bedside B-ultrasound in patients with severe neurological conditions.Methods:A total of 70 patients with severe neurological conditions admitted to the neurosurgery department and intensive care unit of our hospital from September 2023 to August 2024 were selected and divided into control group and experimental group according to random number table method,with 35 cases in each group.The success rate of one puncture,operation time,intraoperative blood loss and related complications of the two groups were compared.Results:The success rate of puncture in experimental group was higher than that in control group(P<0.05).Operation time and blood loss in experimental group were lower than those in control group(P<0.05).The incidence of perioperative complications in experimental group was lower than that in control group(x^(2)=3.968,P=0.046).The long-term complication rate of the experimental group was lower than that of the control group(x^(2)=6.437,P=0.011).Conclusion:The application effect of percutaneous dilatation tracheotomy guided by bedside B-ultrasound is good in the treatment of patients with severe neurological conditions,which can effectively improve the success rate of puncture,shorten the operation time,reduce intraoperative blood loss,avoid tissue damage,and reduce the incidence of complications,with high advantages,and can be promoted and implemented in future clinical treatment.
