摘 要:目的探讨基于《国际功能、残疾和健康分类》(ICF)脑卒中核心分类组合评价虚拟现实(VR)技术训练联合电子生物反馈治疗对脑卒中患者偏瘫侧上肢功能的康复效果。方法前瞻性选取2022年1月至2024年4月合肥市第一人民医院康复医学科收治的62例脑卒中后偏瘫患者为研究对象,研究过程中主动退出训练2例,最终纳入60例。采用随机数字表法分为试验组和对照组,各30例。两组均予以常规康复训练,对照组在常规康复训练基础上联合电子生物反馈治疗,试验组在对照组基础上联合VR技术训练,共干预1个月。比较两组患者ICF脑卒中核心分类组合中上肢功能部分限定值、Fugl-Meyer量表的上肢功能(FMA-UE)评分、组块测试(BBT)、改良Barthel指数(MBI)、上肢Brunnstrom分期及屈肌组、伸肌组肌张力(采用改良Ashworth分级评估)的变化。结果干预后,两组患者关节活动功能、肌张力、手和手臂的使用、精巧手的使用限定值均低于干预前,FMA-UE评分、BBT和MBI均高于干预前,Brunnstrom分期均优于干预前,屈肌组、伸肌组肌张力均优于干预,且试验组上述指标均优于对照组,差异均有统计学意义(均P<0.05)。结论VR技术训练联合电子生物反馈治疗能加强脑卒中患者偏瘫侧上肢功能康复效果,促进ICF脑卒中核心分类组合限定值的下降,改善上肢关节活动度,提高患者生活能力。Objective To evaluate the affected upper limb function rehabilitation in stroke patients with hemiplegia by virtual reality(VR)technology training and electronic biofeedback therapy based on International Classification of Functioning,Disability and Health(ICF)Core Set for Stroke.Methods A prospective study was conducted on 62 patients with post-stroke hemiplegia admitted to Department of Rehabilitation Medicine,Hefei First People's Hospital from January 2022 to April 2024.Two patients withdrew from training during the study and 60 patients were finally enrolled.They were divided into the experimental group and the control group,with 30 cases in each group.Both groups were given conventional rehabilitation training.On this basis,the control group received electronic biofeedback therapy and the experimental group received VR technology training and electronic biofeedback therapy.All patients were given one month of intervention.The changes of the upper limb function in ICF Core Set for Stroke,Fugl-Meyer Assessment-Upper Extremity(FMA-UE)scores,box and block test(BBT),modified Barthel index(MBI)scores,Brunnstrom staging of upper limb,and muscle tone of the flexor group and the extensor group(evaluated by Modified Ashworth Scale)were compared between the two groups.Results After intervention,the limit values of joint motion function,muscle tone,hand and arm use,and fine hand use in both groups were lower than those before intervention,the FMA-UE scores,BBT and MBI were higher than those before intervention,and Brunnstrom staging was better,muscle tone of the flexor group and the extensor group also showed better performance than those before intervention.The above indicators in the experimental group were all superior to those of the control group,and the differences were statistically significant(all P<0.05).Conclusion VR technology training combined with electronic biofeedback therapy can enhance the rehabilitation of upper limb function in stroke patients with hemiplegia,promote the reduction of limit values in I
