基于积极心理学的术前动机性访谈在剖宫产手术患者中的应用效果分析

摘　　要：目的探讨基于积极心理学的术前动机性访谈在剖宫产手术患者中的应用效果。方法选取2022年5月~2024年5月在医院产科进行剖宫产手术的120例产妇为研究对象,按照随机数字表法将产妇随机分成两组,每组60例。对照组产妇接受常规护理,研究组产妇在常规护理中增加基于积极心理学的术前动机性访谈护理。比较两组产妇的心理评分、自我效能感评分、母亲角色适应能力评分、产后泌乳开始时间、泌乳量以及母乳喂养成功率。结果护理后,两组产妇的焦虑和抑郁评分均较护理前显著降低,且研究组产妇焦虑和抑郁评分均低于对照组(P<0.001)。同时,两组自我效能感、母亲角色适应能力评分均较护理前提高,且研究组均高于对照组(P<0.001)。在产后,研究组的泌乳开始时间早于对照组(P<0.001),且产后第1天、第2天、第3天研究组的泌乳量、母乳喂养成功率均高于对照组(P<0.05)。结论在剖宫产手术围手术期护理中,基于积极心理学对产妇进行术前动机性访谈,能够改善产妇的心理状态,增强其自我效能感和母亲角色适应能力,有助于促进产后乳汁分泌,提高母乳喂养率。Objective To explore the application effect of preoperative motivational interviewing based on positive psychology in patients undergoing cesarean section surgery.Methods From May 2022 to May 2024,120 parturients who underwent cesarean section surgery in the hospital's obstetrics department were included.They were randomly divided into two groups,with 60 parturients in each group,using a random number method.The control group of parturients received routine care,while the study group of parturients added preoperative motivational visit nursing based on positive psychology in their care.Compare the psychological scores,self-efficacy scores,maternal role adaptation scores,postpartum lactation start time,lactation volume,and breastfeeding success rate between two groups.Results After nursing care,the anxiety and depression scores of both groups of postpartum women decreased compared to before nursing care,while the psychological scores of the study group were lower than those of the control group(P<0.001).After nursing care,the scores of self-efficacy and maternal role adaptation increased in both groups compared to before nursing care,while the study group was higher than the control group(P<0.001).After childbirth,the lactation start time of the study group was earlier than that of the control group(P<0.001),and the lactation volume and breastfeeding success rate of the study group were higher than those of the control group on the 1st,2nd,and 3rd day postpartum(P<0.05).Conclusion In the perioperative care of cesarean section surgery,motivational interviewing to postpartum women based on positive psychology can improve their psychological state,enhance their self-efficacy and maternal role adaptation ability,promote postpartum milk secretion,and increase breastfeeding rates.
