肝“体阴用阳”理论与肝硬化渐进性进展解析

摘　　要：[目的]解析肝硬化的发病机制,为临床治疗提供思路。[方法]通过对肝“体阴用阳”理论的阐述以及对“取象比类”思维方式的运用,解析肝硬化进展的宏观与微观机制。[结果]肝硬化是多种肝脏疾病的终末期阶段,肝体用失司是肝硬化渐进性进展的重要病理机制,宏观上可以导致气滞、水停、血瘀等病机变化,微观上可以导致肝细胞的变性坏死、细胞外基质(extracellular matrix,ECM)的形成、肝窦内皮细胞(liver sinusoidal endothelial cell,LSEC)窗口的减少、肝窦毛细血管化等病理机制。这些是肝硬化进展的重要因素。因此,基于肝“体阴用阳”这一理论,治疗肝硬化患者可采用滋阴疏肝、活血化瘀兼益气健脾等治法。[结论]肝“体阴用阳”理论是对肝脏生理特征的高度概括,对临床诊疗肝硬化具有启迪作用。[Objective Objective]To analyze the pathogenesis of liver cirrhosis and provide ideas for clinical treatment.[Methods Methods]Through the exposition of the theory of“liver essence Yin and function Yang”and the application of“taking images and comparing classes”thinking mode,the macroscopic and microcosmic mechanism of liver cirrhosis progression was analyzed.[Results Results]Liver cirrhosis is the final stage of many liver diseases.Liver apraxia is an important pathological mechanism of progressive progression of liver cirrhosis.Macroscopically,it can lead to pathological changes such as Qi stagnation,water stagnation,blood stasis and so on.Microscopically,it can lead to the degeneration and necrosis of hepatocytes,the formation of extracellular matrix(ECM),the reduction of liver sinusoidal endothelial cell(LSEC)window,sinusoidal capillarization and other pathological mechanisms.These are important factors in the progression of liver cirrhosis.Therefore,based on the theory of“liver essence Yin and function Yang”,the methods of nourishing Yin and soothing the liver,activating blood circulation and removing blood stasis,invigorating Qi and spleen can be adopted in the treatment of patients with liver cirrhosis.[Conclusion Conclusion]The theory of liver essence Yin and function Yang is a high summary of the physiological characteristics of the liver and has an enlightening effect on the clinical diagnosis and treatment of liver cirrhosis.
