基于郑钦安卢铸之医学理论探讨阳虚型肺炎支原体肺炎的施治思路

摘　　要：肺炎支原体肺炎是一种常见呼吸道感染性疾病,近年来发病率不断攀升。郑钦安卢铸之医学,作为中医学的重要流派,历经百年发展,形成了独特的“切脉-立法-选药-出方”治病体系。本文基于郑钦安卢铸之医学理论,深入探讨阳虚型肺炎支原体肺炎的辨证施治之道。研究认为,该病症的根源在于脾肾阳虚,正气不足,伤于寒邪,外感邪气搏结肺络,即伤寒之证。在治疗上,遵循郑钦安卢铸之医学施治思维“有邪祛邪、无邪建中、纳下填精”的次第法则,辨证性选用桂枝法、附子法、附桂法及非附桂法,旨在祛邪健中,培元固本。治疗原则当以扶正祛邪,补肾填精为本,兼顾祛邪健中,围绕肺、脾、肾三脏,通过扶阳抑阴、用阳化阴等方法,最终实现邪消正足的治疗目的与固守正气的致中和状态。本研究为阳虚型肺炎支原体肺炎的治疗提供了新的思路和方法,有助于缩短疾病周期,减少复发率,提升患者的生活质量。Mycoplasma pneumoniae pneumonia is a common respiratory infectious disease,and its incidence has been increasing in recent years.Zheng Qin'an Lu Zhu's medicine,as an important school of traditional Chinese medicine,has formed a unique“pulse cutting-legislation-drug selection-prescription”treatment system after a hundred years of development.Based on the medical theory of Zheng Qinan and Lu Zhu,this review discusses the dialectical treatment of mycoplasma pneumoniae pneumonia with yang deficiency.The study believes that the root cause of the disease lies in the deficiency of spleen and kidney yang,the lack of healthy qi,the injury of cold evil,and the exogenous evil qi fighting the lung collaterals,that is,the syndrome of typhoid fever.In the treatment,follow Zheng Qinan Lu Zhu's medical treatment thinking“there is evil to dispel evil,no evil to build the middle,and accept the next filling essence”,dialectical selection of Guizhi method,Fuzi method,Fugui method and non-Fugui method,aiming to dispel evil and strengthen the middle,cultivate the yuan and consolidate the foundation.The treatment principle should be based on strengthening the body resistance and eliminating evil,tonifying the kidney and filling the essence,taking into account the elimination of evil and strengthening the middle,focusing on the three organs of the lung,spleen and kidney,through the methods of strengthening yang and suppressing yin,using yang to transform yin,and finally realizing the treatment purpose of eliminating evil and strengthening the body resistance and the neutralization state of adhering to the healthy qi.This study provides new ideas and methods for the treatment of mycoplasma pneumoniae pneumonia with yang deficiency,which is helpful to shorten the disease cycle,reduce the recurrence rate and improve the quality of life of patients.
