血清sEndoglin、RCAS1、CYFRA21-1水平与食管癌患者临床病理特征的相关性分析及临床价值

摘　　要：目的:观察食管癌患者血清可溶性内皮糖蛋白(Soluble endoglin,sEndoglin)、SiSo细胞表达的受体结合肿瘤抗原(Receptor binding cancer antigen expressed on siso cells,RCAS1)、细胞角化素蛋白19片段(Cytokeratin19fragmentantigen21-1,Cyfra21-1)水平,并探究其与临床病理特征相关性及其临床应用价值。方法:选取2020年2月~2023年2月本院108例食管癌患者为研究组,另选取同期健康志愿者54例作为对照组。比较两组血清sEndoglin、RCAS1、CYFRA21-1水平。分析研究组不同临床病理特征者血清sEndoglin、RCAS1、CYFRA21-1水平,并分析其与临床病理特征相关性。评价血清sEndoglin、RCAS1、CYFRA21-1对食管癌的诊断价值。COX回归分析食管癌患者1年内预后的影响因素。结果:研究组血清sEndoglin、RCAS1、CYFRA21-1水平高于对照组(P<0.05);血清sEndoglin、RCAS1、CYFRA21-1与分化程度呈负相关,与淋巴结转移、TNM分期呈正相关(P<0.05);血清sEndoglin、RCAS1、CYFRA21-1联合诊断食管癌的AUC大于单项指标诊断(P<0.05);淋巴结转移、TNM分期及血清sEndoglin、RCAS1、CYFRA21-1水平为预后的影响因素(P<0.05)。结论:血清sEndoglin、RCAS1、CYFRA21-1水平与食管癌患者分化程度呈负相关,与淋巴结转移、TNM分期呈正相关,其对食管癌具有一定诊断价值,且为预后的影响因素,可能作为评估患者预后的潜在指标。Objective:To observe the levels of serum soluble endoglin(sEndoglin),receptor binding cancer antigen expressed on siso cells(RCAS1)and cytokeratin19fragmentantigen21-1(CYFRA21-1)in patients with esophageal cancer,and to explore their correlation with clinicopathological characteristics and clinical application value.Methods:A total of 108 patients with esophageal cancer in our hospital from February 2020 to February 2023 were selected as the study group,and 54 healthy volunteers in the same period were selected as the control group.The serum levels of sEndoglin,RCAS1 and CYFRA21-1 were compared between the two groups.The serum levels of sEndoglin,RCAS1 and CYFRA21-1 in patients with different clinicopathological characteristics in the study group were analyzed,and their correlation with clinicopathological characteristics was analyzed.The diagnostic value of serum sEndoglin,RCAS1 and CYFRA21-1 were evaluated for esophageal cancer.COX regression analysis was used to analyze the prognostic factors of esophageal cancer patients within one year.Results:The serum levels of sEndoglin,RCAS1 and CYFRA21-1 in the study group were higher than those in the control group(P<0.05);the levels of serum sEndoglin,RCAS1 and CYFRA21-1 were negatively correlated with the degree of differentiation,and positively correlated with lymph node metastasis and TNM stage(P<0.05);the AUC of serum sEndoglin,RCAS1,CYFRA21-1 combined diagnosis of esophageal cancer was greater than that of single index diagnosis(P<0.05);lymph node metastasis,TNM stage and serum levels of sEndoglin,RCAS1 and CYFRA21-1 were influencing factors of prognosis(P<0.05).Conclusion:The levels of serum sEndoglin,RCAS1 and CYFRA21-1 are negatively correlated with the degree of differentiation of patients with esophageal cancer,and positively correlated with lymph node metastasis and TNM stage.They have certain diagnostic value for esophageal cancer,and are influencing factors of prognosis,which may be used as potential indicators to evaluate the prognosis of patients.
