摘　　要：目的:分析套筒冠种植覆盖义齿与Locator式种植覆盖义齿修复上颌无牙颌的效果对比。方法:选取2021年2月至2022年3月我院收治的160例上颌无牙颌患者,随机分为对照组(采用Locator式种植覆盖义齿修复)和观察组(采用套筒冠种植覆盖义齿修复),治疗1 y后采用口腔健康影响程度量表(Oral health impact profile,OHIP-14)评估患者的口腔健康程度,采用视觉模拟分析法判定患者的满意度,同时在治疗后1 y评估两组患者的治疗总有效率和菌斑指数、咀嚼效率水平改善。结果:观察组上颌无牙颌患者对舒适度、语音功能、固位稳定功能、咀嚼功能等方面的满意度、修复总有效率显著高于对照组(P<0.05),两组患者治疗前口腔疼痛、独立能力、身体机能、心理状况等各项OHIP-14评分和菌斑指数、咀嚼效率水平相比无显著差异(P>0.05),治疗后均显著改善,且观察组上颌无牙颌患者口腔疼痛、独立能力、身体机能、心理状况等各项OHIP-14评分、菌斑指数、咀嚼效率水平改善优于对照组(P<0.05)。结论:套筒冠种植覆盖义齿修复上颌无牙颌的临床效果优于Locator式种植覆盖义齿。Objective:To analyze the effects of telescopic crown implant-supported overdentures and Locator implant-supported overdentures on the restoration of maxillary edentulous jaws.Method:One hundred and sixty patients with maxillary edentulous jaws admitted to our hospital from February 2021 to March 2022 were randomly divided into a control group(using Locator implant-supported overdentures for restoration)and an observation group(using telescopic crown implant-supported overdentures for restoration).After 1 year of treatment,the patients'oral health status was evaluated using the Oral Health Impact Profile(OHIP-14),and their satisfaction was determined using visual analog scale analysis.At the same time,the overall treatment efficacy,plaque index,and chewing efficiency levels of the two groups were evaluated after 1 year of treatment.Results:The satisfaction and overall treatment efficacy of patients with maxillary edentulous jaws in the observation group in terms of comfort,speech function,retention stability,and chewing function were significantly higher than those in the control group(P<0.05).There were no significant differences in OHIP-14 scores and plaque index and chewing efficiency levels between the two groups before treatment in terms of oral pain,independence,physical function,and psychological status(P>0.05),and all improved significantly after treatment.The improvement in OHIP-14 scores,plaque index,and chewing efficiency levels in patients with maxillary edentulous jaws in the observation group was superior to that in the control group(P<0.05).Conclusion:The clinical effect of telescopic crown implant-supported overdentures for the restoration of maxillary edentulous jaws is superior to that of Locator implant-supported overdentures.
