摘　　要：目的:探讨利培酮、喹硫平与氯氮平治疗精神分裂症的效果差异。方法:选取2020年1月至2023年10月期间于三门峡市康复医院接受治疗的63例精神分裂症患者为研究对象,用随机数字表法分为利培酮组、喹硫平组和氯氮平组,各21例。治疗6m后,比较三组临床疗效,治疗前、治疗6m后,比较三组糖代谢、脂代谢水平,比较治疗期间心电图异常发生情况。结果:三组患者的临床疗效无明显差异(P>0.05)。治疗6 m后,三组外周血HbA1c及血清FINS、TG、HDL水平均明显升高(P<0.05),但三组间无明显差异(P>0.05)。治疗6 m后,利培酮组、氯氮平组血清FPG、2 h PG、TC、LDL均明显升高且明显高于喹硫平组(P<0.05);喹硫平组血清FPG、2 h PG、TC、LDL水平与治疗前相比无明显变化(P>0.05)。治疗期间三组各项心电图异常发生情况无明显差异(P>0.05)。结论:利培酮、喹硫平与氯氮平治疗精神分裂症疗效均较好,喹硫平对糖、脂代谢的影响更小。Objective:To explore the effect difference of risperidone,quetiapine and clozapine in the treatment of schizophrenia.Methods:A total of 63 patients with schizophrenia who were treated in Sanmenxia Rehabilitation Hospital from January 2020 to October 2023 were selected as the research objects.They were divided into the risperidone group,quetiapine group and clozapine group by random number table method,with 21 cases in each group.After 6 months of treatment,the clinical efficacy of the three groups was compared.Before treatment and after 6 months of treatment,the levels of glucose metabolism and lipid metabolism of the three groups were compared,and the occurrence of ECG abnormalities during treatment was compared.Results:There was no significant difference in the clinical efficacy among the three groups(P>0.05).After 6 months of treatment,the levels of HbA1c in peripheral blood and serum FINS,TG and HDL were significantly increased of the three groups(P<0.05).After 6 months of treatment,serum FPG,2 h PG,TC and LDL of the risperidone group and the clozapine group were significantly higher than those of the quetiapine group(P<0.05).During the treatment,there was no significant difference in the occurrence of ECG abnormalities among the three groups(P>0.05).Conclusion:Risperidone,quetiapine and clozapine are effective in the treatment of schizophrenia.Quetiapine has less effect on glucose metabolism and lipid metabolism.
