血清NCAM、Netrin-1在首发精神分裂症患者中的表达及与PANSS评分的相关性

摘　　要：目的检测首发精神分裂症患者血清神经细胞黏附分子(NCAM)、神经轴突导向因子-1(Netrin-1)的表达水平,并探讨其与阳性和阴性综合征量表(PANSS)评分的相关性。方法回顾性选取2022年1月至2023年6月期间驻马店市第二人民医院收治的128例首发精神分裂症患者作为研究组,按蒙特利尔认知评估量表(MoCA)评分将患者分为认知功能障碍组(n=50),认知功能正常组(n=78),并选择同期本院体检健康者110例作为对照组。比较两组受检者的血清NCAM、Netrin-1水平,并比较认知功能障碍组和认知功能正常组患者的血清NCAM、Netrin-1水平及PANSS评分(阳性症状量表、阴性症状量表、一般精神病理量表及总分)。采用Pearson相关分析法分析首发精神分裂症患者血清NCAM、Netrin-1水平与PANSS评分的相关性。结果研究组患者的血清NCAM、Netrin-1水平分别为(81.53±20.20)μg/L、(355.74±54.44)μg/L,明显低于对照组的(134.30±25.65)μg/L、(417.76±60.03)μg/L,差异均有统计学意义(P<0.05);认知功能障碍组患者血清NCAM、Netrin-1水平分别为(59.23±17.37)μg/L、(270.37±18.10)μg/L,明显低于认知功能正常组患者的(96.40±21.66)μg/L、(330.45±25.28)μg/L,差异均有统计学意义(P<0.05);认知功能障碍组患者阳性症状量表、阴性症状量表、一般精神病理量表、PANSS总分分别为(32.33±2.85)分、(29.90±2.91)分、(84.64±3.82)分、(146.87±9.58)分,明显高于认知功能正常组患者的(26.36±3.01)分、(23.38±3.56)分、(76.55±4.53)分、(126.29±11.10)分,差异均有统计学意义(P<0.05);Pearson相关分析结果显示,首发精神分裂症患者血清NCAM、Netrin-1水平与阳性症状量表、阴性症状量表、一般精神病理量表、PANSS总分均呈负相关(P<0.05)。结论首发精神分裂症患者血清NCAM和Netrin-1水平显著降低,且与患者的临床症状严重程度PANSS评分呈负相关,诱发认知功能障碍加重。Objective To detect the expression levels of serum nerve cell adhesion molecule(NCAM)and nerve axon guiding factor Netrin-1 in patients with first-episode schizophrenia,and to explore their correlation with Positive and Negative Syndrome Scale(PANSS)scores.Methods A retrospective study was conducted,enrolling 128 patients with first-episode schizophrenia admitted to the Second People's Medical Hospital of Zhumadian City from January 2022 to June 2023(the study group).Based on their Montreal Cognitive Assessment(MoCA)scores,patients were divided into a cognitive impairment group(n=50)and a cognitively normal group(n=78).Additionally,110 healthy individuals who underwent physical examinations at the same hospital during the same period were selected as the control group.The serum levels of NCAM and Netrin-1 were compared between the study group and the control group,and serum levels of NCAM,Netrin-1,and PANSS scores(positive symptom scale,negative symptom scale,general psychopathology scale,total score)were also compared between the cognitive impairment group and the cognitively normal group.Pearson correlation analysis was used to analyze the correlation between serum NCAM,Netrin-1 levels,and PANSS scores in first-episode schizophrenia patients.Results The serum NCAM and Netrin-1 levels in the study group were(81.53±20.20)μg/L and(355.74±54.44)μg/L,respectively,which were significantly lower than corresponding(134.30±25.65)μg/L and(417.76±60.03)μg/L in the control group,with statistically significant differences(P<0.05).The serum levels of NCAM and Netrin-1 in the cognitive impairment group were(59.23±17.37)μg/L and(270.37±18.10)μg/L,respectively,which were significantly lower than corresponding(96.40±21.66)μg/L and(330.45±25.28)μg/L in the cognitively normal group,with statistically significant differences(P<0.05).The positive symptom scale,negative symptom scale,general psychopathological scale,and PANSS total scores in the cognitive impairment group were(32.33±2.85)points,(29.90±2.91)points,(8
