液基薄层细胞学检测联合DNA倍体细胞检测筛查宫颈癌的价值

摘  要:目的:探讨液基薄层细胞学检测(TCT)联合DNA倍体细胞检测筛查宫颈癌的价值。方法:选取2020年1月—2022年12月于扬州大学附属江都人民医院就诊的196例疑似宫颈癌患者为研究对象,均行TCT检测、DNA倍体细胞检测及阴道镜病理活检。以病理活检结果为“金标准”,分析TCT、DNA倍体细胞检测单独及联合诊断宫颈癌的效能及在宫颈不同程度病变中的检出率。结果:DNA倍体细胞检测与联合检测诊断宫颈癌的准确度、灵敏度高于TCT检测,联合检测灵敏度高于DNA倍体细胞检测(P<0.05);联合检测诊断宫颈上皮内瘤变1级、宫颈上皮内瘤变2级的灵敏度高于TCT检测及DNA倍体细胞检测,联合检测诊断宫颈上皮内瘤变3级的灵敏度高于TCT检测,DNA倍体细胞检测诊断宫颈上皮内瘤变1级、宫颈上皮内瘤变2级、宫颈上皮内瘤变3级的灵敏度高于TCT检测(P<0.05)。结论:TCT联合DNA倍体细胞检测筛查宫颈癌的价值较高,能为临床诊断及治疗提供依据。Objective:To explore the value of liquid-based thin-layer cytology(TCT)combined with DNA ploidy cell detection in screening cervical cancer.Methods:A total of 196 patients with suspected cervical cancer who visited Jiangdu People's Hospital Affiliated to Yangzhou University from January 2020 to December 2022 were selected as the research subjects.All patients underwent TCT detection,DNA ploidy cell detection and colposcopy pathological biopsy.Taking the pathological biopsy results as the"gold standard",the efficacy of TCT and DNA ploidy cell detection alone and in combination for diagnosing cervical cancer and the detection rate in different degrees of cervical lesions were analyzed.Results:The accuracy and sensitivity of DNA ploidy cell detection and combined detection in diagnosing cervical cancer were higher than those of TCT detection,and the sensitivity of combined detection was higher than that of DNA ploidy cell detection(P<0.05).The sensitivity of combined detection in diagnosing cervical intraepithelial neoplasia grade 1 and cervical intraepithelial neoplasia grade 2 was higher than that of TCT detection and DNA ploidy cell detection.The sensitivity of combined detection in diagnosing cervical intraepithelial neoplasia grade 3 was higher than that of TCT detection.The sensitivity of DNA ploidy cell detection in diagnosing cervical intraepithelial neoplasia grade 1,cervical intraepithelial neoplasia grade 2 and cervical intraepithelial neoplasia grade 3 was higher than that of TCT detection(P<0.05).Conclusion:TCT combined with DNA ploidy cell detection has a high value in screening cervical cancer and can provide a basis for clinical diagnosis and treatment.
