针刀联合节律运动对运动神经元性瘫痪患者神经功能和步态的影响

摘　　要：目的探讨针刀联合节律运动对运动神经元性瘫痪患者神经功能和步态的影响。方法选取安阳市中医院2021年5月至2023年5月收治的运动神经元性瘫痪患者93例,按照随机数字表进行分组,对照组46例采用节律运动康复训练,观察组47例在对照组基础上联合针刀疗法,探讨两组治疗效果。结果治疗后,两组美国国立卫生研究院卒中量表(NIHSS)评分均明显下降,且观察组低于对照组,组间比较,差异有统计学意义(P<0.05)。治疗后,观察组腱反射评分高于对照组,肌张力、踝阵挛、临床痉挛指数(CSI)总评分均低于对照组,组间比较,差异有统计学意义(P<0.05)。观察组治疗后步频、步速、步幅水平均上升,高于对照组,差异有统计学意义(P<0.05)。观察组总有效率95.74%高于对照组78.26%,差异有统计学意义(P<0.05)。结论针刀联合节律运动能够有效改善患者的神经功能状况,提高肌肉力量和协调性,对运动神经元性瘫痪患者的步态恢复具有积极的影响。Objective To explore the effect of acupotomy combined with rhythmic movement on nerve function and gait in patients with motor neuron paralysis.Methods Ninety-three patients with motor neuron paralysis admitted to Anyang Hospital of Traditional Chinese Medicine from May 2021 to May 2023 were divided into groups according to random number table,46 patients in the control group received rhythmic movement rehabilitation training,and 47 patients in the observation group combined with acupotomy therapy on the basis of the control group.Results After treatment,the score of the National Institute of Health stroke scale(NIHSS)in both groups decreased significantly,and the observation group was lower than the control group,with difference of statistical significance between two groups(P<0.05).After treatment,tendon reflex score of the observation group was higher than the control group,muscle tone,ankle clonus and clinic spastcity index(CSI)score were lower than the control group,and the difference was statistically significant(P<0.05).After treatment,the level of step frequency,step speed and stride length in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The clinical effect of the observation group 95.74%was higher than that of the control group 78.26%,and the difference was statistically significant(P<0.05).Conclusion Acupotomy combined with rhythmic movement can effectively improve the nerve function of patients,improve muscle strength and coordination,and have a positive impact on the gait recovery of patients with motor neuron paralysis.
