摘 要:目的探讨静态调强(Step&Shoot)子野权重优化(SWO)技术对直肠癌患者新辅助放疗计划剂量学指标的影响。方法选取2022年1月至2024年3月我院收治的82例直肠癌患者为研究对象,依据在放疗计划中采用的优化技术不同分为两组各41例。两组均接受调强放疗(IMRT),对照组采用Step&Shoot方式对IMRT计划进行优化,观察组在对照组基础上采用SWO技术进行优化。比较两组IMRT计划中的机器跳数(MU)及总子野数,采用剂量体积直方图评估两组的靶区适形指数(CI)、均匀性指数(HI)及危及器官(OAR)的剂量分布并比较。结果观察组的MU较对照组高,总子野数较对照组低(P<0.05)。两组左右侧股骨头接受V_(30)、V_(40)、V_(50)剂量的体积及小肠接受V_(40)、V_(50)剂量的体积比较,差异均无统计学意义(P>0.05);观察组小肠接受V_(30)剂量的体积及膀胱接受V_(30)、V_(40)、V_(50)剂量的体积均较对照组低(P<0.05)。两组的靶区CI及HI值比较,差异无统计学意义(P>0.05)。结论在Step&Shoot优化直肠癌新辅助放疗计划的基础上采用SWO技术进行优化可减少总子野数,有利于缩短放疗时间;此外,还能够降低正常组织照射剂量,有利于降低患者所受到的放疗毒副作用,保障治疗安全性。Objective To explore the impact of static intensity modulation(Step&Shot)segment weight optimization(SWO)technique on the dosimetric indicators of neoadjuvant radiotherapy plan for rectal cancer patients.Methods 82 patients with rectal cancer admitted to our hospital from January 2022 to March 2024 were selected as the research subjects,and divided into two groups based on the optimization techniques used in the radiotherapy plan,with 41 cases in each group.Both groups received intensity-modulated radiation therapy(IMRT).The control group optimized the IMRT plan us ing the Step&Shot method,while the observation group optimized the IMRT plan using the SWO technique on the basis of the control group.The monitor unit(MU)and total subfields in IMRT plan were compared between the two groups.The target area conformity index(CI)and homogeneity index(HI),and dose distribution of organs at risk(OAR)were evaluated by dose volume histogram and compared between the two groups.Results The MU of the observation group was higher than that of the control group,and the total number of subfields was lower than that of the control group(P<0.05).No statistically significant difference was found in the volume of the left and right femoral heads receiving V_(30),V_(40),and V_(50)doses,as well as the volume of the small intestine receiving V_(40)and Vso doses between the two groups(P>0.05);The volume of the small intestine receiving V_(30)dose and the volume of the bladder receiving V_(30),V_(40),and V_(50)doses in the observation group were lower than those in the control group(P<0.05).No statistically significant difference was found in the CI and HI values of target area between the two groups(P>0.05).Conclusions On the basis of optimizing the neoadjuvant radiotherapy plan for rectal cancer by Step&Shot,using SWO technology for optimization can reduce the total number of subfields and shorten the radio therapy time;In addition,it can also reduce the normal tissue irradiation dose,which is beneficial for reducing the toxic side effec
