微脉冲激光光凝联合玻璃体视网膜手术及雷珠单抗治疗增殖期糖尿病性视网膜病变患者的效果观察

摘  要:目的分析微脉冲激光光凝联合玻璃体视网膜手术及雷珠单抗治疗增殖期糖尿病性视网膜病变(PDR)患者的效果。方法选取我院2023年4月至2024年4月收治的PDR患者90例,随机分为两组各45例。对照组予以玻璃体腔注射雷珠单抗注射液配合玻璃体视网膜手术治疗,观察组在对照组基础上予以577 nm阈值下微脉冲激光光凝治疗。比较两组的最佳矫正视力(BCVA)、黄斑中心凹视网膜厚度(CMT)、血清血管内皮生长因子(VEGF)、深层毛细血管层(DCP)血流密度、生活质量以及并发症。结果治疗后,观察组的BCVA显著高于对照组,CMT显著低于对照组(P<0.05)。治疗后,观察组的血清VEGF显著低于对照组,DCP血流密度显著高于对照组(P<0.05)。治疗后,观察组的SF-36量表各维度评分均显著高于对照组(P<0.05)。观察组的并发症发生率为4.44%,显著低于对照组的20.00%(P<0.05)。结论577 nm阈值下微脉冲激光光凝联合玻璃体视网膜手术及雷珠单抗能够有效调节PDR患者的BCVA、CMT、VEGF及DCP血流密度,降低并发症发生率,提高患者生活质量。Objective To analyze the effect of micropulse laser photocoagulation combined with vitreoretinal surgery and ranibizumab in the treatment of patients with proliferative diabetic retinopathy(PDR).Methods 90 patients with PDR admitted to our hospital from April 2023 to April 2024 were selected and randomly divided into two groups,with 45 cases in each group.The control group was treated with intravitreal injection of ranibizumab injection combined with vitreoretinal surgery,and the observation group was treated with micropulse laser photocoagulation under 577 nm threshold on the basis of the control group.The best corrected visual acuity(BCVA),central macular thickness(CMT),serum vascular endothelial growth factor(VEGF),deep capillary layer(DCP)blood flow density,quality of life and complications were compared between the two groups.Results After treatment,BCVA of the observation group was significantly higher than that of the control group,and CMT was significantly lower than that of the control group(P<0.05).After treatment,the serum VEGF of the observation group was significantly lower than that of the control group,and the DCP blood flow density was significantly higher than that of the control group(P<0.05).After treatment,the scores of each dimension of SF-36 scale in the observation group were significantly higher than those in the control group(P<0.05).The incidence of complications in the observation group was 4.44%,significantly lower than 20.00%in the control group(P<0.05).Conclusions Micropulse laser photocoagulation under 577 nm threshold combined with vitreoretinal surgery and ranibizum ab can effectively regulate BCVA,CMT,VEGF and DCP blood flow density in patients with PDR,reduce the incidence of complications and improve the quality of life of patients.
