超声骨刀联合反角高速涡轮对下颌低位阻生齿患者术后疼痛及张口受限程度的影响

摘　　要：目的探讨超声骨刀联合反角高速涡轮对下颌低位阻生齿患者术后疼痛及张口受限程度的影响。方法选取医院2020年7月至2022年7月期间收入的107名患者作为研究对象,采用随机数表法将其分为对照组(n=53,采用反角高速涡轮法)和观察组(n=54,采用超声骨刀+反角高速涡轮)。比较两组围手术期情况,比较术后第3、5、7天两组疼痛、张口受限、面部肿胀情况,比较两组炎症因子水平[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和超敏C反应蛋白(hs-CRP)],比较两组术后并发症。结果观察组术中出血量比对照组低,手术时间比对照组短(P<0.05);术后第3、5、7天,两组VAS评分逐渐降低,张口受限、面部肿胀占比逐渐减少,且各时点观察组VAS评分比对照组低,张口受限、面部肿胀占比比对照组低(P<0.05);术后第3天,两组IL-6、TNF-α和hs-CRP水平比术前高,但观察组比对照组低(P<0.05);观察组术后并发症发生率比对照组低(P<0.05)。结论超声骨刀联合反角高速涡轮治疗下颌低位阻生齿患者,可减少出血量、缩短手术时间,减轻患者疼痛、张口受限、面部肿胀及炎症反应,且术后并发症少。Objective To explore the effect of ultrasonic osteotome combined with high-speed turbine with dihedral angle on postoperative pain and mouth opening limitation of patients with low mandibular impacted teeth.Methods 107 patients admitted to our hospital from July,2020 to July,2022 were selected as the research objects,and they were randomly divided into control group that 53 patients were treated with anti-angle high-speed turbine and observation group which that 54 patients were treated with ultrasonic osteotome and anti-angle high-speed turbine.Compared the perioperative situation between two groups,compared the pain,restricted mouth opening,and facial swelling on the 3rd,5th,and 7th day after surgery,compared the levels of inflammatory factors that interleukin-6(IL-6),tumor necrosis factor alpha(TNF alpha),and hypersensitive C-reactive protein(hs-CRP)between the two groups,and compared the postoperative complications between the two groups.Results The intraoperative bleeding in the observation group was lower than that in the control group,and the surgical time was shorter than that in the control group(P<0.05).On the 3rd,5th,and 7th day after surgery,the VAS scores of both groups gradually decreased,and the proportion of restricted mouth opening and facial swelling gradually decreased.At each time point,the VAS scores of the observation group were lower than those of the control group,and the proportion of restricted mouth opening and facial swelling was lower than that of the control group(P<0.05).On the third day after surgery,the levels of IL-6,TNF-α,and hs CRP in both groups were higher than before surgery,but the observation group was lower than the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion Ultrasonic osteotome combined with reverse angle high-speed turbine can reduce bleeding,shorten operation time,and relieve pain,limited mouth opening,facial swelling and inflammatory reaction,and there are few
