摘 要:目的比较口外支抗与微螺钉种植体支抗治疗成人安氏Ⅱ类1分类错[牙合]畸形患者的效果及对咀嚼功能的影响。方法选择2019年1月至2022年1月收治的84例成人安氏Ⅱ类1分类错[牙合]畸形患者,采用随机数字表法将其分为甲组、乙组,各42例。甲组予以口外支抗治疗,乙组予以微螺钉种植体支抗治疗。比较两组的治疗效果。结果乙组的治疗总有效率高于甲组,并发症总发生率低于甲组(P<0.05)。治疗后,两组的翼上颌裂后缘垂线至上颌磨牙远中邻面的垂直距离(Ptm-U6)、上中切牙切缘至上槽牙座点连线的垂直距离(U1-NA)及Apg线至上中切牙切缘的垂直距离(Apg-U1)均低于治疗前,且乙组低于甲组(P<0.05)。乙组的正畸疗程短于甲组(P<0.05);治疗后,两组的咀嚼效率高于治疗前,且乙组高于甲组(P<0.05)。结论相较于口外支抗,微螺钉种植体支抗治疗安氏Ⅱ类1分类错[牙合]畸形可显著提高疗效,降低并发症发生率,缩短正畸治疗时间,改善头影测量指标及咀嚼功能,值得在临床中推广及应用。Objective To compare the effect of extraoral anchorage and micro-screw implant anchorage in the treatment of adult patients with Angle classⅡdivision 1 malocclusion deformity and its influence on masticatory function.Methods From January 2019 to January 2022,84 adult patients with Angle classⅡdivision 1 malocclusion deformity were selected and divided into group A and group B by random number table method,with 42 cases in each group.The group A was treated with extraoral anchorage,and the group B was treated with micro-screw implant anchorage.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the group B was higher than that in the group A,and the total incidence of complications was lower than that in the group A(P<0.05).After treatment,the vertical distance from the posterior edge of the pterygomaxillary fissure to the distal surface of the maxillary molar(Ptm-U6),the vertical distance from the cutting edge of the upper central incisor to the connection line of the upper alveolar seat(U1-NA)and the vertical distance from the Apg line to the cutting edge of the upper central incisor(Apg-U1)in the two groups were lower than those before treatment,and those in the group B were lower than those in the group A(P<0.05).The orthodontic treatment course of the group B was shorter than that of the group A(P<0.05);after treatment,the chewing efficiency of the two groups was higher than that before treatment,and that of the group B was higher than that of the group A(P<0.05).Conclusion Compared with extraoral anchorage,micro-screw implant anchorage in the treatment of Angle classⅡdivision 1 malocclusion deformity can significantly improve the curative effect,reduce the incidence of complications,shorten the orthodontic treatment time,improve the cephalometric indexes and masticatory function,which is worthy of promotion and application in clinical practice.
