区域性入肝血流阻断在原发性肝癌腹腔镜肝切除术治疗中的应用效果

摘　　要：目的探讨分析区域性入肝血流阻断(RHIB)在原发性肝癌(PCL)腹腔镜肝切除术(LH)治疗中的应用效果。方法选取2020年1月至2023年12月在阳新县人民医院肝胆外科治疗的60例PCL-LH患者,随机分为对照组与观察组,各30例。观察组采用RHIB治疗,对照组采用间歇性全入肝血流阻断(ITHO)治疗。对比两组手术指标、肝功能指标、并发症发生率。结果两组的手术时间、住院时间对比,P>0.05;观察组肝血流阻断时间、术中出血量均少于对照组,P<0.05;术前两组肝功能指标对比,P>0.05;术后5 d观察组ALT、AST、TBil均低于对照组,ALB高于对照组,P<0.05。两组并发症发生率对比,P>0.05。结论RHIB用于PCL-LH治疗中具有较高的安全性与有效性,可显著改善患者肝功能,促进患者尽早康复。Objective:To explore and analyze the effect of regional hepatic inflow occlusion(RHIB)in laparoscopic hepatectomy(LH)for primary liver cancer(PCL).Methods:Sixty patients with PCL-LH who were treated in hepatobiliary surgery in Yangxin People's Hospital from January 2020 to December 2023 were randomly divided into control group and observation group,with 30 patients in each group.The observation group was treated with RHIB,while the control group was treated with intermittent total hepatic blood flow occlusion(ITHO).The surgical indexes,liver function indexes and the incidence of complications were compared between the two groups.Results:The operation time and hospitalization time of the two groups were compared,P>0.05.The hepatic blood flow blocking time and intraoperative blood loss in the observation group were less than those in the control group,P<0.05.The comparison of liver function indexes between the two groups before operation,P>0.05;5 days after operation,ALT、AST and TBil in the observation group were lower than those in the control group,while ALB was higher than that in the control group,P<0.05.The incidence of complications between the two groups was compared,P>0.05.Conclusion:RHIB is safe and effective in the treatment of PCLLH,which can significantly improve patients'liver function and promote patients'early recovery.
