摘  要:目的 通过有限元分析法来研究种植体螺纹长宽比与形状对周围骨质的影响。方法 运用SolidWorks 21.0以straumannBLT 4.1×10 mm种植体为原型,并将螺纹的长宽比和螺纹的形状作为变量设计种植体,对1例65岁下颌无牙颌男性患者的口腔颌面锥形束CT(cone beam computer tomography, CBCT)进行提取分析,且利用ANSYS17.0分析软件模拟正常咬合对种植体施加轴向载荷,采用ANSYS 17.0分析软件对种植体的Von Mises应力分布进行分析。结果 种植体的螺纹深度对周围骨骼的影响大于螺纹宽度。螺纹深度与植入骨周围的应力变化呈负相关,而螺纹宽度与植入骨附近的应力改变呈正相关;在四种不同的螺纹形状中,在两种不同的载荷下,V形螺纹形状对周围骨骼产生的应力最低,而圆形V形螺纹形式对周围骨骼的应力最高;在这两种载荷下,皮质骨比小梁骨承受更大的应力。结论 比值0.4∶0.2是所有实验组中种植体应力最低的一组;V形螺纹结构种植体在垂直与斜向载荷下的皮质骨应力最小。Objective To study the influence of the aspect ratio and shape of implant threads on the surrounding bone using finite element analysis method.Methods Using SolidWorks 21.0,the straumannBLT 4.1×10 mm implant was used as the prototype,and the aspect ratio and shape of the thread were used as variables to design the implant.A CBCT scan of a 65 year old male patient with edentulous mandible was extracted and analyzed.ANSYS 17.0 analysis software was used to simulate the axial load applied to the implant during normal occlusion.ANSYS 17.0 analysis software was used to analyze the Von Mises stress distribution of the im⁃plant.Results The thread depth of the implant had a greater impact on the surrounding bone than the thread width.The thread depth was negatively correlated with the stress change around the implanted bone,while the thread width was positively correlated with the stress change near the implanted bone;among the four different thread shapes,the V⁃shaped thread shape pro⁃duced the lowest stress on the surrounding bone under two different loads,while the circular V⁃shaped thread form produced the highest stress on the surrounding bone;under both loads,the cortical bone born greater stress than the trabecular bone.Conclusion The ratio of 0.4∶0.2 is the group with the lowest implant stress among all experimental groups;The minimum corti⁃cal bone stress of V⁃shaped threaded implant under vertical and oblique loads.
