摘 要:目的调查天津市滨海新区碘缺乏地区8~10岁儿童碘营养状况,以及年龄、身高和体重对甲状腺容积的影响。方法于2023年,在天津市滨海新区碘缺乏地区采取现况调查的方法,覆盖全部乡(镇、街道办事处)抽取200名8~10岁儿童为调查对象,收集儿童尿样及其家中食用盐样品,检测尿碘水平和盐碘含量;并进行甲状腺容积检测和体格检查。结果天津市滨海新区200名8~10岁儿童家庭食用盐盐碘中位数25.08mg/kg;尿碘中位数125.04μg/L,8岁、9岁和10岁儿童尿碘中位数分别是119.70μg/L、147.01μg/L、121.95μg/L,不同年龄段儿童尿碘水平差异没有统计学意义(H=0.488,P>0.05);男生的尿碘中位数154.29μg/L,女生的尿碘中位数为104.65μg/L,不同性别儿童尿碘水平差异有统计学意义(Z=-3.202,P<0.05);儿童甲状腺肿大率4.5%(9/200),不同年龄、性别的儿童甲状腺肿大率差异均无统计学意义(P>0.05);年龄、身高、体重、体质指数、体表面积与甲状腺容积均呈正相关(rs分别为0.241、0.371、0.340、0.236、0.361)。多重线性回归分析结果显示,年龄、身高均与甲状腺容积呈正相关关系(P<0.05),年龄和身高的标准化偏回归系数分别为0.167、0.209,提示身高对甲状腺容积的影响大于年龄对甲状腺容积的影响。结论天津市滨海新区儿童碘营养处于适宜状态,甲状腺肿大率符合国家规定的碘缺乏病控制消除标准的要求,儿童甲状腺容积的评价应综合考虑儿童体格发育指标。Objective To understand the iodine nutrition status of children aged 8-10 in iodine deficient area of Binhai New Area of Tianjin,and the effects of age,height and weight on thyroid volume.Methods In 2023,a current situation survey was conducted in iodine deficient area of Binhai New Area of Tianjin,covering all townships(towns,street offices)and 200 children aged 8-10 were chosen as the subjects.The urine samples and household salt samples were collected to determine urinary iodine levels and salt iodine content,and thyroid volume testing and physical examination were conducted.Results The median iodine content of salt and iodine consumed by 200 children aged 8-10 in Binhai New Area of Tianjin was 25.08 mg/kg,the median urinary iodine level was 125.04μg/L,the median urinary iodine levels for children aged 8,9,and 10 were 119.70μg/L,147.01μg/L and 121.95μg/L,respectively.There was no statistically significant difference in urinary iodine levels among children of different age groups(P>0.05).The median urinary iodine levels for boys were 154.29μg/L,and for girls were 104.65μg/L.There was a statistically significant difference in urinary iodine levels between children of different genders(P<0.05).The thyroid enlargement rate in children was 4.5%(9/200),and there was no statistically significant difference in thyroid enlargement rates among children of different ages and genders(P>0.05).Age,height,weight,body mass index,body surface area and thyroid volume were all positively correlated(Spearman correlation coefficients are 0.241,0.371,0.340,0.236 and 0.361,respectively).Multiple linear regression results showed that age and height were positively correlated with thyroid volume(P<0.05),and the standardized partial regression coefficients for age and height were 0.167 and 0.209,respectively,indicating that height had a greater effect on thyroid volume than age.Conclusion The iodine nutrition of children in Binhai New Area of Tianjin is in a suitable state,and the goiter rate meets the requirements of the nationa
