中药熏蒸及重复经颅磁刺激联合针刺对脑卒中后中枢性面瘫疗效观察

摘　　要：目的:分析中药熏蒸及重复经颅磁刺激(rTMS)联合针刺对脑卒中后中枢性面瘫的疗效。方法:共纳入新余钢铁集团有限公司中心医院2021年6月至2024年2月期间收治的120例脑卒中后中枢性面瘫患者,以随机数表法分为A组、B组、C组及对照组,各30例。对照组患者予以针刺治疗,A组患者予以中药熏蒸联合针刺治疗,B组患者予以rTMS联合针刺治疗,C组患者予以中药熏蒸、rTM、针刺联合治疗。比较4组患者疗效。于治疗前后比较4组患者面瘫程度[House-Brackmann(H-B)面神经分级功能值]、面神经功能[面部残疾指数(FDI)评分]、面部肌群恢复状况[采用表面肌电图监测患侧面肌群均方根(RMS)值]。结果:C组患者总有效率高于A组、B组及对照组,差异具有统计学意义(P<0.05);治疗后C组患者H-B分级的功能值均较其余各组高,FDI评分均较其余各组低,差异具有统计学意义(P<0.05);治疗后C组患者面部肌群RMS值均较其余各组高,差异具有统计学意义(P<0.05)。结论:中药熏蒸及rTMS联合针刺应用于脑卒中后中枢性面瘫患者中,可提高疗效,减轻面瘫程度,提高面神经功能,促进面部肌群恢复。Objective To analyze the efficacy of traditional Chinese medicine steaming and repetitive transcranial magnetic stimulation(rTMS)combined with acupuncture in the treatment of central facial palsy after stroke.Methods A total of 120 patients with central facial palsy stroke admitted to the Central Hospital of Xinyu Iron and Steel Group Co.,Ltd.from June 2021 to February 2024 were included and randomly divided into Group A,Group B,Group C,and a control group,with 30 cases in each group.The control group received acupuncture treatment,Group A received traditional Chinese medicine steaming combined with acupuncture,Group B received rTMS combined with acupuncture,and Group C received a combination of traditional Chinese medicine steaming,rTMS,and acupuncture.The efficacy of the four groups was compared.The degree of facial palsy[House–Brackmann(H–B)facial nerve grading function scale],facial nerve function[facial disability index(FDI)score],and facial muscle group recovery[surface electromyography to monitor the root mean square(RMS)value of the affected facial muscle group]were compared before and after treatment.Results The total effective rate in Group C was higher than that in Group A,Group B,and the control group,with statistically significant differences(P<0.05).After treatment,the H–B functional value in Group C was higher than that in the other groups,and the FDI score was lower than that in the other groups,with statistically significant differences(P<0.05).After treatment,the RMS values of the facial muscle group in Group C were higher than those in the other groups,with statistically significant differences(P<0.05).Conclusion The application of traditional Chinese medicine steaming and rTMS combined with acupuncture in patients with central facial palsy after stroke can improve the efficacy,reduce the degree of facial palsy,enhance facial nerve function,and promote the recovery of facial muscle groups.
