介入栓塞联合大分割三维适形放疗治疗不可切除肝癌的临床疗效

摘　　要：目的:分析不可切除原发性肝癌(PHC)予以肝动脉化疗栓塞术(TACE)联合大分割三维适形放疗方案的临床疗效。方法:选取南阳市中心医院2022年1月至2024年1月收治的不可切除PHC患者80例,根据随机数字表法分为对照组(TACE治疗)和观察组(TACE联合大分割三维适形放疗治疗),各40例。比较两组患者治疗后的临床疗效、实验室指标(血清趋化因子9、热休克蛋白90)水平及不良反应发生情况。结果:与对照组比较,观察组患者治疗总有效率较高,治疗后血清趋化因子9和热休克蛋白90水平较低,差异均具有统计学意义(P<0.05);两组患者在治疗期间不良反应发生率比较,差异无统计学意义(P>0.05)。结论:不可切除PHC患者接受TACE治疗基础上联合大分割三维适形放疗方案,能够下调血清趋化因子9和热休克蛋白90水平,提高疾病控制有效率,且不会增加治疗不良反应。Objective To analyze the clinical efficacy of hepatic arterial chemoembolization(TACE)combined with large segment three-dimensional conformal radiotherapy for unresectable primary liver cancer(PHC).Methods A total of 80 patients with unresectable PHC admitted to Nanyang Central Hospital from January 2022 to January 2024 were selected and divided into a control group(TACE treatment)and an observation group(TACE combined with large segment three-dimensional conformal radiotherapy)according to random number table method,with 40 patients in each group.The clinical efficacy,laboratory indexes(serum chemokine 9,heat shock protein 90)and the occurrence of adverse reactions were compared between the two groups after treatment.Results Compared with the control group,the effective rate of treatment in the observation group was higher,and the levels of chemokine 9 and heat shock protein 90 after treatment were lower,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups during treatment(P>0.05).Conclusion The combination of TACE treatment and large segment three-dimensional conformal radiotherapy in unresectable PHC patients can downregulate serum chemokine 9 and heat shock protein 90 levels,improve disease control efficacy,and not increase treatment adverse reactions.
