摘 要:目的探讨去整合素-金属蛋白酶17(ADAM17)、Notch1在子宫内膜异位症(EMs)中的表达及其与上皮-间质转化(EMT)的关系。方法选取2018年10月至2022年12月就诊于潍坊医学院附属医院患有EMs卵巢异位囊肿者病人40例,在行腹腔镜或开腹时剥除的囊壁内侧的异位出血病灶作为EMs异位内膜组(n=40),术中收取其在位子宫内膜作为EMs在位内膜组(n=40),选取同期因妇科其他良性疾病来院就诊的病人40例,在行诊刮术或内膜活检时收取其子宫内膜作为正常子宫内膜组(对照组)。采用免疫组织化学染色法检测EMs异位内膜、在位内膜及正常子宫内膜组中ADAM17、Notch1、E-钙黏蛋白、N-钙黏蛋白的定位与表达情况;结果ADAM17、Notch1、N-钙黏蛋白在EMs异位内膜组表达评分5.00(4.00,6.00)分、4.5(4.0,5.0)分、6.0(5.0,7.0)分最高,EMs在位内膜组表达评分4.00(3.00,4.80)分、4.0(3.0,5.0)分、5.0(4.0,6.0)分次之,正常子宫内膜组表达评分2.00(2.00,3.00)分、3.0(2.0,4.0)分、2.5(2.0,3.0)分最低,均差异有统计学意义(P<0.05);E-钙黏蛋白在EMs异位内膜组表达评分1.0(1.0,2.0)分最低,EMs在位内膜组表达评分3.0(2.0,4.0)分次之,正常子宫内膜组表达评分5.5(5.0,6.0)分最高,均差异有◇临床医学◇·550·安徽医药Anhui Medical and Pharmaceutical Journal 2025 Mar,29(3)统计学意义(P<0.05);正常内膜组中各项表达评分均差异无统计学意义(P>0.05);EMs异位内膜组中ADAM17与Notch1的表达呈正相关(rs=0.46,P=0.003),Notch1与N-钙黏蛋白的表达呈正相关(rs=0.38,P=0.014),E-钙黏蛋白与ADAM17、Notch1的表达分别呈负相关(rs=-0.36,P=0.024;rs=-0.44,P=0.004),余下均差异无统计学意义(P>0.05);EMs在位内膜组中,ADAM17与Notch1、N-钙黏蛋白的表达分别呈正相关(rs=0.34,P=0.031;rs=0.61,P<0.001),Notch1与N-钙黏蛋白的表达呈正相关(rs=0.63,P<0.001);E-钙黏蛋白与ADAM17、Notch1、N-钙黏蛋白的表达均呈负相关(rs=-0.51,P=0.001;rs=-0.32,P=0.041;rs=Objective To explore the expressions of a disintegrin and metalloprotease 17(ADAM17)and Notch1 in endometriosis(EMs),as well as their relationship with epithelial-to-mesenchymal transition(EMT).Methods Forty patients with endometriosis who were treated in The Affiliated Hospital of Weifang Medical College from October 2018 to December 2022 were selected for the study.The ovarian endometriotic cysts on the inner wall of the uterus,which were performed laparoscopic or open surgery,were included in the EMs ectopic endometrium group(n=40),the in situ endometriums collected during the procedures as the EMs eutopic endometrium group(n=40),and endometriums obtained during curettage or endometrial biopsy from other 40 patients who came to the hospital for other benign gynecological diseases during the same period as the normal endometrium group(control group).The localization and expression levels of ADAM17,Notch1,E-cadherin and N-cadherin in ectopic endometrium,eutopic endometrium and normal endometrium were detected by immunohistochemical staining.Results The expression scores of ADAM17,Notch1 and N-cadherin were the highest in the EMs ectopic endometrium group[5.00(4.00,6.00),4.5(4.0,5.0),6.0(5.0,7.0)],followed by those in the EMs eutopic endometrium group[4.00(3.00,4.80),4.0(3.0,5.0),5.0(4.0,6.0)],and the normal endometrium group[2.00(2.00,3.00),3.0(2.0,4.0),2.5(2.0,3.0)],and the difference was statistically significant(P<0.05).The expression score of E-cadherin was the lowest in the EMs ectopic endometrium group[1.0(1.0,2.0)],that in the EMs eutopic endometrium group was[3.0(2.0,4.0)],and the highest was in the normal endometrium group[5.5(5.0,6.0)];the difference was statistically significant(P<0.05).There were no significant differences in expression scores in the normal endometrium group(P>0.05).In the EMs ectopic endometrium group,there was a positive correlation between the expressions of ADAM17 and Notch1(rs=0.46,P=0.003),a positive correlation between Notch1 and N-cadherin(rs=0.38,P=0.014),and a negative corr
