摘  要:目的初步探讨彩色多普勒血流分级与周围型肺癌微血管密度的相关性及线性关系,为后续超声及时评价动态监测肺癌疗效的进一步研究打下基础。方法回顾性分析2020年1月至2022年3月广州医科大学附属第一医院105例行超声引导肺穿刺活检证实为肺癌的病人相关资料。在穿刺活检前对病灶行彩色多普勒血流显像(CDFI),并将病灶血流丰富程度分为0~Ⅲ级;随后对病灶行超声引导下的穿刺活检,对所取标本行组织病理学检测及CD34免疫组织化学染色,计数病灶的微血管密度(MVD),并探讨CDFI血流分级与MVD的相关性及线性关系。结果105例病人中,病灶最大径:≤5 cm者34例、>5 cm者71例;CDFI血流分级:0级4例、Ⅰ级25例、Ⅱ级28例、Ⅲ级48例;Mann-Whitney U检验显示,≤5 cm组与>5 cm组MVD差异无统计学意义;Spearman相关分析显示血流分级与MVD具有正相关关系(r_(s)=0.31,P<0.001);简单线性回归分析示血流分级与MVD存在线性关系,(F=14.55,P<0.001),回归方程:MVD=18.349+4.990×血流分级(r^(2)=0.12)。结论彩色多普勒血流分级与周围型肺癌MVD呈正相关,有望成为一项无创性、及时评价、动态监测肺癌疗效的指标。Objective To discuss the correlations between color Doppler blood flow grades and micro vessel density(MVD)in peripheral lung cancer,to lay a foundation for the further research on the timely evaluation and dynamic monitoring of the therapeutic effect of lung cancer by using ultrasound.Methods The relevant data of 105 patients diagnosed with lung cancer through ultrasound-guided lung biopsy at the First Affiliated Hospital of Guangzhou Medical University from January 2020 to March 2022 were retrospectively analyzed.Color Doppler flow imaging(CDFI)examination was performed on the lesion before biopsy procedure,and the blood flow of the lesion was graded into 4 grades(from grade 0 to grade Ⅲ);and then ultrasound guided lung biopsy was performed on the lesion,the samples of the lesion were sent to made histopathological as well as CD34 immunohistochemical examinations,the microvessel density(MVD)of the lesion was counted,and then the correlations between blood flow grades and MVD in peripheral lung cancer were discussed.Results Among 105 cases,there were 34 cases with the maximum diameter of the lesion being≤5 cm,and 71 cases being>5 cm.The distribution of blood flow grades:4 cases with grade 0,25 cases with grade Ⅰ,28 cases with grade Ⅱ,48 cases with grade Ⅲ.Mann-Whitney U test showed that there was no significant differences in MVD between the≤5 cm group and>5 cm group;Spearman correlation analysis showed that there were positive correlations between blood flow grades and MVD(r_(s)=0.31,P<0.001).Simple linear regression analysis showed that there were linear relations between blood flow grades and MVD(F=14.55,P<0.001),regression equation:MVD=18.349+4.990×blood flow grade(r^(2)=0.12).Conclusion There is a positive correlation between color Doppler blood flow grades and MVD in peripheral lung cancer,indicating that the blood flow grades may become a noninvasive,timely evaluation and dynamic monitoring index of therapeutic effect in lung cancer.
