微针疗法治疗紧张型头痛的研究进展

摘　　要：紧张型头痛(TTH)是一种反复发作的内科疾病,发病率逐年升高。通过检索近年来微针疗法治疗TTH的文献,发现微针疗法中的头针、耳穴、眼针、腹针、腕踝针、足针均对TTH患者的头痛程度以及其伴随症状如焦虑、抑郁、睡眠障碍等有较好疗效。其中头针刺激量大,可改善脑部血流、调控神经递质;耳穴临床使用广泛,可通过神经-内脏途径以及调控抗痛物质发挥镇痛作用;眼针可通过激活细胞外信号与调控脑部血流从而治疗TTH;腹针既可缓解TTH患者颈部肌肉紧张,又可通过脑肠轴刺激腹部神经发挥镇痛作用;部分临床试验已证明腕踝针与足针的镇痛效果。但目前在治疗TTH方面,腕踝针与足针治疗TTH的作用机制研究较少,需要更多实验研究以弥补腕踝针和足针治疗TTH的机制;同时也需要更多临床研究丰富选穴的多样性。Tension type headache(TTH)is a recurrent internal medicine disease,and its incidence is increasing year by year.By searching literature on the treatment of TTH with microneedle therapy in recent years,it is found that the microneedle therapy,including head acupuncture,ear acupuncture,eye acupuncture,abdominal acupuncture,wrist ankle acupuncture,and foot acupuncture has good therapeutic effects on the degree of headache and accompanying symptoms such as anxiety,depression,and sleep disorders in TTH patients.Among them,head acupuncture has a large stimulation amount,which can improve cerebral blood flow and regulate neurotransmitters;Ear acupoints are widely used in clinical practice,and can exert analgesic effects through the neurovisceral pathway and regulating analgesic substances;Eye acupuncture can treat TTH by activating extracellular signals and regulating cerebral blood flow;Abdominal acupuncture can not only relieve neck muscle tension in TTH patients,but also stimulate abdominal nerves through the brain gut axis to exert analgesic effects;Partial clinical trials have demonstrated the analgesic effects of wrist ankle acupuncture and foot acupuncture.However,currently there is limited research on the mechanism of action of wrist ankle acupuncture and foot acupuncture in the treatment of TTH,and more experimental researches are needed to compensate for the mechanism of wrist ankle acupuncture and foot acupuncture in the treatment of TTH;At the same time,more clinical researches are needed to enrich the diversity of acupoint selection.
