摘　　要：胆道癌恶性程度高,根治性手术是治愈胆道癌的唯一潜在治疗手段。由于起病隐匿,绝大部分患者在诊断时即为晚期,失去根治性手术机会。转化治疗可使初始不可切除肿瘤降期并达到R0切除,使患者获得手术机会,延长生存,已被应用于多种晚期肿瘤治疗,并展现良好潜力。但胆道癌转化治疗尚处起步阶段,关于转化人群筛选、转化策略制定、转化手术时机把握等诸多问题亟待深入探索。在此背景下,本文将结合团队所在单位的临床经验及相关研究进展,阐述晚期胆道癌转化手术策略,旨在为胆道癌转化治疗提供参考。Biliary tract cancer(BTC)is highly malignant,with curative surgical resection being the only potentially definitive treatment.Due to its frequently asymptomatic onset,most patients are diagnosed at an advanced stage,precluding the opportunity for curative surgery.Conversion therapy offers the potential to downstage initially unresectable tumors,thereby making R0 resection possible,providing patients with surgical opportunities,and potentially extending survival.This approach has shown promising results in various advanced cancers.However,the application of conversion therapy in BTC is still in its nascent stages,with unresolved issues related to patient selection,treatment strategies,and the timing of conversion surgery.In this context,this article reviews conversion therapy strategies for advanced BTC,drawing on clinical experience and recent research from our institution and aiming to provide valuable insights and guidance for the field.
