摘 要:目的探究癌胚抗原相关细胞黏附分子19抗体(CEACAM19)通过调控PI3K/AKT信号通路对胃癌发病及预后影响,为临床诊断及预后判断提供参考。方法采用免疫组织化学(IHC)染色技术检测CEACAM19和AKT蛋白在胃癌患者胃癌组织及相应癌旁组织中的表达水平。同时采用荧光定量PCR检测CEACAM19、AKT的mRNA表达水平。分析、比较CEACAM19、AKT表达与胃癌患者临床病理特征的关系,并通过Pearson和Spearman相关性分析以及Kaplan-Meier生存曲线分析探讨二者之间的关联及其对预后的影响。结果CMACAM19、AKT蛋白在胃癌组织中的阳性表达率明显高于癌旁组织(78.75%比17.50%,76.25%比15.00%,P<0.05)。胃癌组织中CEACAM19、AKT mRNA表达水平均明显高于癌旁组织(P<0.05)。CEACAM19、AKT阳性表达在TNM分期、分化程度、浸润程度以及淋巴结转移上存在显著差异(P<0.05)。CEACAM19与AKT的蛋白及mRNA表达呈现正相关(r=0.715,P=0.000;r=0.557,P=0.000)。Kaplan-Meier生存分析显示,CEACAM19、AKT阳性表达患者的总体生存期、无病生存期均短于阴性表达者,经Log-rank检验差异有统计学意义(χ^(2)=8.054、8.660、7.815、8.721,P=0.005、0.003、0.005、0.003)。结论CEACAM19基因及其调控的PI3K/AKT信号通路在胃癌的发生、发展过程中扮演重要角色,CEACAM19、AKT蛋白可能作为胃癌进展的生物标志物,在促进胃癌的侵袭性和恶性进展中发挥重要作用。Objective To research the influence of carcinoembryonic antigen associated cell adhesion molecule 19 antibody(CEACAM19)gene on the pathogenesis and prognosis of gastric cancer by regulating PI3K/AKT signaling pathway,and then provide reference for clinical diagnosis and prognosis judgment of gastric cancer.Methods Immunohistochemical(IHC)staining technique was used to detect the expression levels of CEACAM19 and AKT protein in gastric cancer tissues and corresponding adjacent tissues of patients with gastric cancer.At the same time,the mRNA expression levels of CEACAM19 and AKT were detected by fluorescence quantitative PCR.The relationships between the expression of CEACAM19 and AKT and the clinicopathological features of gastric cancer patients were analyzed and compared,and the relationship between them and their influence on prognosis was explored by Pearson and Spearman correlation analysis and Kaplan-Meier survival curve analysis.Results The positive expression rates of CMACAM19 and AKT protein in gastric cancer tissues were significantly higher than those in adjacent tissues(78.75%vs.17.50%,76.25%vs.15.00%,P<0.05).The expression levels of CEACAM19 and AKT mRNA in gastric cancer tissues were significantly higher than those in adjacent tissues(P<0.05).The positive expressions of CEACAM19 and AKT were significantly different in TNM stage,differentiation degree,infiltration degree and lymph node metastasis(P<0.05).CEACAM19 was positively correlated with the protein and mRNA expression of AKT(r=0.715,P=0.000;r=0.557,P=0.000).Kaplan-Meier survival analysis showed that the overall survival and disease-free survival of patients with positive expression of CEACAM19 and AKT were lower than those with negative expression,and the differences were statistically significant by Log-rank test(χ^(2)=8.054,8.660,7.815,8.721,P=0.005,0.003,0.005,0.003).Conclusion CEACAM19 gene and its regulated PI3K/AKT signaling pathway play an important role in the occurrence and development of gastric cancer.CEACAM19 and AKT protein may b
