不同手术方案治疗闭角型青光眼合并白内障的疗效分析

摘　　要：目的分析不同手术方案治疗闭角型青光眼合并白内障的疗效。方法选取122例单眼闭角型青光眼合并白内障患者,随机分为对照组及观察组,每组61例。对照组采取白内障超声乳化+小梁切除术,观察组采取白内障超声乳化+房角分离术。比较两组术后视力状况,术后并发症发生情况,手术前后的眼压、前房深度。结果两组术后视力提高率比较,差异无意义(P>0.05)。观察组术后并发症发生率4.9%低于对照组的16.4%,差异有意义(P<0.05)。手术前,两组的眼压、前房深度比较,差异无意义(P>0.05);手术后,观察组的眼压(13.3±2.3)mm Hg(1 mm Hg=0.133 kPa)、前房深度(3.1±0.5)mm低于对照组的(17.4±3.6)mm Hg、(3.3±0.6)mm,差异有意义(P<0.05)。结论两种术式均可改善患者的视力状况,其中白内障超声乳化+房角分离术的并发症少,可有效降低患者术后眼压、前房深度,可推广使用。Objective To analyze the curative effect of different surgical schemes on angle-closure glaucoma combined with cataract.Methods A total of 122 patients with monocular angle-closure glaucoma combined with cataract were selected,and randomly divided into control group and observation group,each consisting of 61 cases.The control group was treated with cataract phacoemulsification+trabeculectomy,and the observation group was treated with cataract phacoemulsification+goniosynechialysis.The postoperative visual acuity,postoperative complications,intraocular pressure and anterior chamber depth before and after operation were compared between the two groups.Results There was no significant difference in the improvement rate of postoperative visual acuity between the two groups(P>0.05).The incidence of postoperative complications in the observation group was 4.9%,which was lower than 16.4% in the control group,and the difference was significant(P<0.05).Before operation,there was no significant difference in intraocular pressure and anterior chamber depth between the two groups(P>0.05).After operation,the intraocular pressure of the observation group was(13.3±2.3)mm Hg(1 mm Hg=0.133 kPa)and the anterior chamber depth was(3.1±0.5)mm,which were lower than(17.4±3.6)mm Hg and(3.3±0.6)mm of the control group,and the differences were significant(P<0.05).Conclusion Both surgical procedures can improve the visual acuity of patients.Phacoemulsification plus goniosynechialysis has fewer complications and can effectively reduce the postoperative intraocular pressure and anterior chamber depth of patients,so it can be widely used.
