摘 要:目的研讨不同角膜屈光手术对近视患者眼红指数及泪膜破裂时间的影响。方法70例(140眼)近视手术患者,采用随机数字表法分为观察组[35例(70眼),全飞秒小切口角膜基质透镜取出术(SMILE)]与对照组[35例(70眼),半飞秒准分子激光原位角膜磨镶术(LASIK)]。比较两组视力、不同方位角膜知觉阈值、眼红指数及泪膜破裂时间。结果术后30 d,两组视力比较差异无统计学意义(P>0.05)。术后30 d,观察组角膜鼻侧知觉阈值(51.45±10.67)mm、角膜颞侧知觉阈值(53.54±10.43)mm、角膜下方知觉阈值(52.60±6.76)mm、角膜中间知觉阈值(39.22±8.48)mm、角膜上方知觉阈值(48.02±8.23)mm均高于对照组的(40.17±8.30)、(38.62±10.52)、(40.43±10.11)、(34.09±9.58)、(35.31±8.72)mm,差异有统计学意义(P<0.05)。术前及术后7、30 d,两组眼红指数比较差异均无统计学意义(P>0.05)。术前,两组泪膜破裂时间比较差异无统计学意义(P>0.05);术后7、30 d,观察组泪膜破裂时间(7.66±2.01)、(8.87±2.78)s长于对照组的(6.59±1.71)、(7.99±2.12)s,差异有统计学意义(P<0.05)。结论与半飞秒LASIK治疗方案对比,采用全飞秒SMILE治疗近视能够有效改善患者术后角膜知觉、泪膜破裂时间,减少对角膜神经的损伤,值得临床推广应用。Objective To discuss the effect of different corneal refractive surgery on ocular redness index and tear film break-up time in myopia patients.Methods 70 patients(140 eyes)with myopia were divided into observation group[35 cases(70 eyes),full femtosecond small incision lenticule extraction(SMILE)]and control group[35 cases(70 eyes),half femtosecond laser-assisted in situ keratomileusis(LASIK)]by random number table method.The visual acuity,corneal perception threshold in different orientations,ocular redness index and tear film break-up time were compared between the two groups.Results At 30 d after surgery,there was no significant difference in visual acuity between the two groups(P>0.05).At 30 d after surgery,in the observation group,the corneal nasal perception threshold was(51.45±10.67)mm,the corneal temporal perception threshold was(53.54±10.43)mm,the lower corneal perception threshold was(52.60±6.76)mm,the middle corneal perception threshold was(39.22±8.48)mm,and the upper corneal perception threshold was(48.02±8.23)mm,which were higher than(40.17±8.30),(38.62±10.52),(40.43±10.11),(34.09±9.58),and(35.31±8.72)mm in the control group.The difference was statistically significant(P<0.05).There was no significant difference in the ocular redness index between the two groups before surgery and at 7 and 30 d after surgery(P>0.05).Before surgery,there was no significant difference in tear film break-up time between the two groups(P>0.05).At 7 and 30 d after surgery,the tear film break-up time in the observation group were(7.66±2.01)and(8.87±2.78)s,which were longer than(6.59±1.71)and(7.99±2.12)s in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with half femtosecond LASIK,full femtosecond SMILE for myopia patients can effectively improve the postoperative corneal perception and tear film break-up time,reduce the damage to corneal nerve,which is worthy of clinical promotion.
