牙周-牙髓联合病变患者牙周状态及血清IL-6、MMP-8、MMP-9水平表达分析

摘　　要：目的 探讨牙周-牙髓联合病变患者牙周状态及血清白介素6(IL-6)、基质金属蛋白酶8(MMP-8)、MMP-9表达水平意义。方法 选取2020年6月—2022年12月东南大学附属中大医院江北院区收治的牙周-牙髓联合病变患者108例,其中50例接受牙体牙髓及牙周治疗(治疗组),58例未接受治疗(未治疗组),并取同期50例由于正畸拔牙健康者作为对照组,比较3组牙周袋病原菌检出率、原菌感染分布情况、血清IL-6、MMP-8、MMP-9表达水平,使用Logistic回归模型分析血清IL-6、MMP-8、MMP-9与牙周-牙髓联合病变的关系。结果 未治疗组牙周袋病原菌检出率显著高于治疗组、对照组(P<0.05),治疗组与对照组比较差异无统计学意义(P>0.05);未治疗组牙周袋病原菌以福赛类杆菌(27.59%)、中间普氏菌(18.97%)、牙龈卟啉单胞菌(15.52%)为主,且未治疗组福赛类杆菌、中间普氏菌、牙龈卟啉单胞菌检出率显著高于治疗组、对照组(P<0.05);未治疗组血清IL-6、MMP-8、MMP-9表达水平显著高于治疗组、对照组(P<0.05)。Logistic回归分析显示,血清IL-6、MMP-8、MMP-9升高为牙周-牙髓联合病变危险因素(P<0.05)。结论 牙周-牙髓联合病变患者牙周袋具有较高病原菌感染率,以福赛类杆菌、中间普氏菌、牙龈卟啉单胞菌为主,患者血清IL-6、MMP-8、MMP-9表达水平异常升高,为牙周-牙髓联合病变危险因素。Objective To analyze the periodontal status and serum interleukin 6(IL-6),matrix metalloproteinase 8(MMP-8) and MMP-9 levels in patients with combined periodontic-endodontic lesions.Methods A total of 108 patients with combined periodontic-endodontic lesions were selected as the experimental group,including 50 patients receiving dental pulp and periodontal treatment(experimental group 1) and 58 patients without treatment(experimental group 2).Meanwhile,50 healthy individuals who underwent orthodontic tooth extraction were selected as the control group.The detection rate of pathogenic bacteria in periodontal pocket,distribution of pathogenic bacteria,serum IL-6,MMP-8 and MMP-9 levels were compared among the 3 groups.Logistic regression model was used to analyze the relationship between serum IL-6,MMP-8 and MMP-9 and combined periodontic-endodontic lesions.Results The detection rate of pathogenic bacteria in periodontal pocket of the experimental group 2 was significantly higher than that in the experimental group 1 and that in the control group(P<0.05).However,there was no significant difference between the experimental group 1 and the control group(P>0.05).The main pathogenic bacteria in periodontal pocket of the experimental group 2 included Bacteroides forsythus(27.59%),Prevotella intermedia(18.97%) and Porphyromonas gingivalis(15.52%).The detection rates of Bacteroides forsythus,Prevotella intermedia and Porphyromonas gingivalis in the experimental group 2 were significantly higher than those in the experimental group 1 and the control group(P<0.05).Serum IL-6,MMP-8 and MMP-9 levels in the experimental group 2 were significantly higher than those in the experimental group 1 and the control group(P<0.05).Logistic regression analysis found that elevated serum IL-6,MMP-8 and MMP-9 levels were risk factors for combined periodontic-endodontic lesions(P<0.05).Conclusion Patients with combined periodontic-endodontic lesions have a higher incidence of infection.Bacteroides forsythus,Prevotella intermedia and Porphyrom
