摘 要:目的探讨光动力疗法(PDT)对种植患者龈沟液炎性因子水平和菌群多样性的影响。方法选择2023年1月至11月在我院接受种植牙患者94例作为研究对象。所有患者均按照种植牙术后7 d予以1次PDT治疗。收集患者PDT治疗前后种植体龈沟液进行炎性因子水平及菌群多样性检测,并比较不同时间点之间的差异。结果PDT治疗后龈沟液中白介素6(IL-6)、肿瘤坏死因子α(TNF-α)及基质金属蛋白酶-7(MMP-7)均较治疗前明显下降,转化生长因子-β1(TGF-β1)较治疗前明显升高(P<0.05)。PDT治疗后Chao1指数、ACE指数及Shannon指数低于治疗前,而Simpson指数高于治疗前,差异有统计学意义(P<0.05)。β多样性分析结果提示治疗前后龈沟液中菌群特征存在差异。PDT治疗前龈沟液中:普雷沃氏菌属、弧菌属、纤毛菌属等丰度较高,且为主要优势菌;PDT治疗后龈沟液中不动杆菌属、梭杆菌属及奈瑟菌属等丰度较高,为主要优势菌,(LDA>4log10)。结论PDT治疗能有效降低种植牙患者龈沟液中炎性因子,同时可以影响菌群多样性及分布情况。Objective To investigate the effects of photodynamic therapy(PDT)on gingival crevicular fluid inflammatory factors level and microbial diversity in patients with implantation.Methods 94 patients who received dental implants in our hospital from January to November 2023 were selected as the study objects.All patients were treated with PDT once every 7 days after implantation.Gingival crevicular fluid was collected before and after PDT treatment to detect inflammatory factors level and microbial diversity,and the differences between different time points were compared.Results The levels of IL-6,TNF-αand MMP-7 in gingival crevular fluid after PDT treatment were significantly lower than those before treatment,and TGF-β1 was significantly higher than those before treatment(P<0.05).After PDT treatment,Chao1 index,ACE index and Shannon index were lower than before treatment,while Simpson index was higher than before treatment,the difference was statistically significant(P<0.05).The results ofβ-diversity analysis suggested that the microflora characteristics in gingival crevicular fluid were different before and after treatment.Prevotella,Vibrio and Leptothrix were the predominant bacteria in the gingival crevicular fluid before PDT treatment.After PDT treatment,Acinetobacter,Fusobacterium and Neisseria were the main dominant bacteria in the gingival crevicular fluid(LDA>4log10).Conclusion PDT treatment can effectively reduce the inflammatory factors level in gingival crevicular fluid of patients with dental implants,and can affect the diversity and distribution of bacteria.
