经桡动脉与经股动脉途径全脑血管造影对诊断脑血管疾病有效性及安全性对比的Meta分析

摘  要:目的评价经桡动脉途径和经股动脉途径全脑血管造影的效果差异。方法检索万方数据库、CNKI中国知网、维普中文期刊网、中国生物医学文献数据库(CBM)、PubMed、MEDLIN及EMBASE。收集经桡动脉途径和经股动脉途径用于全脑血管造影的临床随机对照研究。使用软件RevMan 5.3进行Meta分析。结果本研究共纳入可研究文献15篇,其中中文文献12篇,外文文献3篇,共4898例患者(经桡动脉途径组2369例,经股动脉途径组2529例)。Meta分析结果显示:经桡动脉途径组有较低的术后假性动脉瘤发生率[RR=0.17,95%CI(0.08~0.36),P<0.00001]、尿潴留发生率[RR=0.16,95%CI(0.07~0.33),P<0.00001]、血肿发生率[RR=0.21,95%CI(0.15~0.31),P<0.00001]。结论经桡动脉途径与经股动脉途径全脑血管造影效果相当,但经桡动脉途径并发症更少更安全,因此本研究认为可优先选择桡动脉途径。Objective To evaluate differences in effects of transradial and transfemoral artery whole brain angiography.Methods Wanfang Database,CNKI,VIP Chinese Journals Network,China Biomedical Literature Database(CBM),PubMed,MEDLIN and EMBASE were searched.Clinical randomized controlled studies of transradial and transfemoral routes for whole-brain angiography were collected.Meta-analysis was performed using RevMan 5.3.Results Total 15 studies were included,including 12 Chinese literature and 3 foreign language literature,with 4898 patients(2369 in transradial group and 2529 in transfemoral group).The incidence of postoperative pseudoaneurysm was/ower in the transradial group[RR=0.17,95%CI(0.08~0.36),P<0.00001],urinary retention[RR=0.16,95%CI(0.07~0.33),P<0.00001],and hematoma[RR=0.21,95%CI(0.15~0.31),P<0.00001].Conclusion Effects of transradial approach would be comparable to that of transfemoral approach,but transradial approach has fewer complications and is safer,so radial approach could be preferred.
