摘  要:双相障碍是一种以抑郁发作与躁狂发作交替出现的慢性精神疾病,其有显著的认知受损,且这种认知损害可从急性发作期持续至缓解期,甚至终生存在,严重影响患者职业功能及生存质量。双相障碍的认知损害机制复杂,是多因素相互作用的结果,涵盖大脑结构异常、代谢性疾病、遗传学和神经回路、脑-肠轴系统、免疫紊乱和炎症等方面。目前对其具体作用机制及相互关系的认识尚不完全明确,亟待进一步深入研究,以探寻更为有效的治疗靶点和干预措施,从而改善双相障碍患者的认知功能及预后。Bipolar disorder is a chronic mental illness characterized by alternating episodes of depression and mania.It is associated with significant cognitive impairment that can persist from the acute phase to the remission phase,and even throughout the patient's life,severely affecting their occupational function and quality of life.The mechanisms underlying cognitive impairment in bipolar disorder are complex and result from the interaction of multiple factors,including brain structural abnormalities,metabolic diseases,genetics and neural circuits,the brain-gut axis system,immune disorders,and inflammation.Currently,there is still a lack of a clear understanding of the specific mechanisms and their interrelationships,which necessitates further in-depth research,so as to discover more effective therapeutic targets and intervention measures to improve the cognitive function and prognosis of the patients with bipolar disorder.
