摘 要:目的探究CBL联合MDT教学法在神经内科住院医师规范化培训带教中的应用。方法选取2022年8月至2023年8月上海中医药大学2022级规培生26人,其中男生10人,女生16人,入科时对相关神经系统疾病进行基础知识测试,按随机数表法将26名住培生随机分为研究组、对照组,各13例。研究组实施MDT联合CBL教学法;对照组实施传统教学法。比较两组临床考核、综合能力评分、学习兴趣及满意度的差异。结果研究组基础理论知识评分、诊疗能力(医疗面谈技能、体格检查技能、人文关怀、临床判断、沟通技能、组织效能、整体表现)评分均高于对照组,差异有统计学意义(P<0.05)。两组教学后分析和创新力得分、医疗文书的书写得分、分析及解决问题能力得分均高于教学前(P<0.05),且研究组均高于对照组,差异有统计学意义(P<0.05)。等级资料秩和检验显示,研究组总体满意度和学习兴趣满意度均高于对照组,差异有统计学意义(P<0.05)。结论CBL联合MDT教学法运用在神经内科住院医师规范化培训教学中是可实施的,且拥有显著的优势,这种教学方法不仅便于提高住院医师的临床诊疗能力和理论知识水准,还能够培养他们的批判性思维能力和团队协作能力,为医师的未来发展规划奠定稳固的基础。Objective To explore the application of CBL combined with MDT teaching method in the standardized training and teaching of neurology residents.Methods A total of 26 students from August 2022 to August 2023 in Shanghai University of Traditional Chinese Medicine,including 10 male students and 16 female students,were randomly divided into the study group and the control group,with 13 cases in each group.The MDT combined with CBL teaching method was implemented in the study group,and the traditional teaching method was implemented in the control group.The differences in clinical assessment,comprehensive ability score,learning interest and satisfaction between the two groups were compared.Results The scores of basic theoretical knowledge and diagnosis and treatment ability(medical interview skills,physical examination skills,humanistic care,clinical judgment,communication skills,organizational effectiveness,and overall performance)in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).The scores of post-teaching analysis and innovation,medical document writing,analysis and problem-solving ability in the two groups were higher than those before teaching(P<0.05),and the research group was higher than that in the control group,and the difference was statistically significant(P<0.05).The rank sum test of grade data showed that the overall satisfaction and learning interest satisfaction of the study group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The CBL combined MDT teaching method can be implemented in the standardized training and teaching of neurology residents,and has significant advantages,which is not only convenient to improve the clinical diagnosis and treatment ability and theoretical knowledge level of residents,but also can cultivate their critical thinking ability and teamwork ability,and lay a solid foundation for the future development planning of physicians.
