自制康复仪在缺血性脑卒中偏瘫并糖尿病患者中的应用效果

摘　　要：目的探讨自制康复仪在缺血性脑卒中偏瘫并糖尿病患者中的应用效果。方法选取2022年2月至2024年2月广州中医药大学深圳医院(福田)收治的86例缺血性脑卒中偏瘫并糖尿病患者作为研究对象,按照随机数字表法将其分为对照组和干预组,每组各43例。对照组采用常规护理,干预组在对照组基础上增加自制康复仪进行抗阻康复训练,比较两组患者的血糖水平[空腹血糖(FPG)、餐后2 h血糖(2 hBG)、糖化血红蛋白(HbA1c)]、神经功能缺损程度[美国国立卫生研究院卒中量表(NIHSS)]、日常生活活动能力(Barthel指数)、独立生活能力(改良Rankin量表)和安全性。结果干预前,两组患者FPG、2 hBG、HbA1c水平,NIHSS、Barthel指数和改良Rankin量表评分比较,差异无统计学意义(P>0.05);干预后,干预组FPG、2 hBG水平,NIHSS、改良Rankin量表评分均低于对照组,Barthel指数评分高于对照组,差异有统计学意义(P<0.05);两组患者干预后HbA1c水平比较,差异无统计学意义(P>0.05)。结论对于缺血性脑卒中偏瘫并糖尿病患者加用自制康复仪进行抗阻康复训练,能减少血糖波动,提高生活质量,但对HbA1c的影响不大,还有待进一步深入研究。Objective To investigate the application effect of self-made rehabilitation instrument in patients with ischemic stroke with hemiplegia and diabetes mellitus.Methods A total of 86 patients with ischemic stroke with hemiplegia and diabetes mellitus admitted to Shenzhen Hospital of Guangzhou University of Chinese Medicine(Futian)from February 2022 to February 2024 were selected as the study objects,and they were divided into control group and intervention group according to random number table method,with 43 cases in each group.The control group received routine nursing,and the intervention group added self-made rehabilitation apparatus for resistance rehabilitation training on the basis of the control group.The blood glucose levels(fasting plasma glucose[FPG],2 hour post-meal blood glucose[2 hBG],glycosylated hemoglobin[HbA1c]),degree of neurological impairment(national institutes of health stroke scale[NIHSS]),ability to perform activities of daily living(Barthel index),ability to live independently(modified Rankin scale),and safety were compared between the two groups.Results Before intervention,there were no significant differences in FPG,2 hBG,HbA1c,NIHSS,Barthel index and modified Rankin scale scores between two groups(P>0.05).After intervention,the levels of FPG,2 hBG,NIHSS and modified Rankin scale in the intervention group were lower than those in the control group,and the Barthel index score was higher than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in HbA1c level between the two groups after intervention(P>0.05).Conclusion For patients with ischemic stroke with hemiplegia and diabetes mellitus,resistance rehabilitation training with self-made rehabilitation instrument can reduce blood glucose fluctuation and improve quality of life,but it has little effect on HbA1c,and further research is needed.
