摘　　要：结直肠侧向发育型肿瘤(LST)是一类发生于结直肠,特征为较大且相对平坦,沿着肠壁呈周围水平扩散的结直肠腺瘤。通常无明显临床症状,由于其独特的生长模式和较高的恶性潜能,给患者的生存质量带来很大影响,漏诊及预后不良时有发生。现代医学研究发现LST与各种基因、细胞因子及通路之间有联系,早期诊断主要依靠电子结肠镜,经上述检查明确诊断后采取内镜下对症治疗,且发现相比其他肠道息肉,LST具有更高的癌变风险,是与早期结肠癌有关的癌前病变,早治疗,好预后便尤为重要。现就其发病机制、分子生物学、西医诊疗方法的最新进展作一综述。Laterally spreading tumors(LST)are a type of colorectal adenoma that occurs in the colon and is characterized by being large and relatively flat,spreading horizontally along the intestinal wall.Usually without obvious clinical symptoms,due to its unique growth pattern and high malignant potential,it has a great impact on the survival and treatment of patients,and missed diagnosis and poor prognosis often occur.Modern medical research has found connections between LST and various genes,cytokines,and pathways.Early diagnosis mainly relies on electronic colonoscopy.After the above examination confirms the diagnosis,targeted treatment under endoscopy is adopted.It has been found that LST has a higher risk of carcinogenesis compared to other intestinal polyps,and is a precancerous lesion related to early colon cancer.Early treatment and good prognosis are particularly important.This article provides a review of the latest developments in its pathogenesis,molecular biology,and Western medicine diagnosis and treatment methods.
