体部伽玛刀立体定向放疗联合吉西他滨、卡培他滨化疗治疗不可手术局部晚期胰腺癌疗效观察

摘　　要：目的探讨体部伽玛刀立体定向放疗联合吉西他滨、卡培他滨化疗治疗不可手术局部晚期胰腺癌的临床效果和安全性。方法选取2015年1月至2021年3月在解放军联勤保障部队第906医院收治的43例不可手术局部晚期胰腺癌患者,按治疗方法的不同将其分为观察组(n=22)与对照组(n=21)。对照组单纯接受吉西他滨、卡培他滨化疗,观察组在对照组化疗基础上联合体部伽玛刀立体定向放疗,比较2组患者的疗效、不良反应。结果观察组总有效率77.2%、疾病控制率95.4%,均高于对照组的38.0%、66.7%(χ^(2)=6.776,P=0.009;χ^(2)=5.889,P=0.015)。2组患者均未出现严重致死性不良反应,出现的轻度不良反应经对症处理后逐渐缓解或自行缓解。观察组预后优于对照组(χ^(2)=4.006,P=0.046)。结论体部伽玛刀立体定向放疗联合吉西他滨、卡培他滨化疗治疗不可手术局部晚期胰腺癌安全有效,值得临床推荐应用。Objective To observe the clinical efficacy and safety of stereotactic body gamma knife radiotherapy combined with gemcitabine and capecitabine chemotherapy in the treatment of unresectable locally advanced pancreatic cancer.Methods Forty-three patients with unresectable locally advanced pancreatic cancer admitted from January 2015 to March 2021 were randomly treated with gemcitabine and capecitabine chemotherapy alone(the control group,n=21)or gemcitabine and capecitabine chemotherapy combined with stereotactic body gamma knife radiotherapy(the observation group,n=22).The efficacy and adverse reactions were compared between the two groups.Results The total effective rate and the disease control rate in the observation group were 77.2%and 95.4%,respectively,both of which were higher than those in the control group(38.0%and 66.7%;χ^(2)=6.776,P=0.009;χ^(2)=5.889,P=0.015).No severe fatal adverse reactions occurred in the two group,and the mild adverse reactions gradually alleviated or resolved spontaneously after symptomatic treatment.The prognosis of the observation group was better than that of the control group(χ^(2)=4.006,P=0.046).Conclusion Stereotactic body gamma knife radiotherapy combined with gemcitabine and capecitabine chemotherapy is safe and effective.
