血清SAA1联合肿瘤标志物水平在胃癌分化及分期评估中的应用

摘　　要：目的研究血清淀粉样蛋白A1(SAA1)联合肿瘤标志物水平在胃癌分化及分期评估中的应用。方法回顾性选取2021年1月至2023年12月在江南大学附属医院就诊的80例胃癌患者纳入研究。根据TNM分期:Ⅰ期12例,Ⅱ期23例,Ⅲ期29例,Ⅳ期16例;根据分化程度:低分化27例,中分化38例,高分化15例。检测不同TNM分期、不同分化程度胃癌患者的血清癌胚抗原(CEA)、糖类抗原(CA)72-4、CA19-9、CA125、SAA1水平;采用Pearson相关性分析对SAA1、肿瘤标志物水平与胃癌分化及分期的相关性进行分析。结果SAA1及CA19-9、CA12、CEA、CA72-4在TNM分期Ⅰ期、Ⅱ期、Ⅲ期、Ⅳ期患者中的表达水平均逐渐增加,差异均有统计学意义(P<0.05)。SAA1及CA19-9、CA12、CEA、CA72-4在低分化、中分化、高分化患者中的表达水平均逐渐下降,差异均有统计学意义(P<0.05)。Pearson相关性分析显示,SAA1及CA19-9、CA12、CEA、CA72-4与TNM分期呈正相关(r=0.547、0.465、0.654、0.468、0.478;P<0.05),与分化程度呈负相关(r=0.423、-0.389、-0.547、-0.647、-0.467;P<0.05)。结论胃癌不同分化程度及TNM分期患者SAA1及肿瘤标志物CA19-9、CA12、CEA、CA72-4水平均存在明显差异,观察以上指标可能为胃癌评估提供依据。Objective To study the application of serum amyloid A1(SAA1)combined with tumor markers in the differentiation and staging evaluation of gastric cancer.Methods A retrospective study was conducted on 80 patients with gastric cancer who were admitted to the Affiliated Hospital of Jiangnan University from January 2021 to December 2023.According to TNM staging,there were 12 cases of stage Ⅰ,23 cases of stage Ⅱ,29 cases of stage Ⅲ and 16 cases of stage Ⅳ;according to degree of differentiation:poorly differentiated of 27 cases,moderately differentiated of 38 cases,and highly differentiated of 15 cases.The levels of serum carcinoembryonic antigen(CEA),carbohydrate antigen(CA)72-4,CA19-9,CA125 and SAA1 were detected in patients with different TNM stages and different degrees of differentiation of gastric cancer.And the correlation between serum SAA1,tumor marker levels and gastric cancer differentiation and staging was analyzed by Pearson correlation analysis.Results The expression levels of SAA1,CA19-9,CA12,CEA and CA72-4 in patients with stageⅠ,Ⅱ,Ⅲ and Ⅳof TNM stage increased gradually,the differences were statistically significant(P<0.05).The expression levels of SAA1,CA19-9,CA12,CEA and CA72-4 decreased gradually in patients with low differentiation,moderate differentiation and high differentiation,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that SAA1,CA19-9,CA12,CEA and CA72-4 were positively correlated with TNM stage(r=0.547,0.465,0.654,0.468,0.478;P<0.05),and negatively correlated with differentiation degree(r=0.423,-0.389,-0.547,-0.647,-0.467;P<0.05).Conclusion There are significant differences in levels of SAA1 and tumor markers CA19-9,CA12,CEA,and CA72-4 among patients with different degrees of differentiation and stages of gastric cancer.Observing these indicators may provide a basis for the evaluation of gastric cancer.
