健脾益气法联合甲地孕酮片治疗结肠癌术后患者化疗后食欲不振的临床观察

摘　　要：目的观察健脾益气法联合甲地孕酮片治疗结肠癌术后患者化疗后食欲不振的临床疗效。方法将60例结肠癌术后化疗患者按照随机数字表法分为2组,对照组30例予口服甲地孕酮片治疗,治疗组30例在对照组基础上予健脾益气法治疗。2组均治疗21 d。比较2组治疗前后中医证候评分、卡氏功能状态评分(KPS)、厌食/恶液质治疗功能评估量表(FAACT)、肿瘤标志物糖类抗原72-4(CA72-4)及炎症因子肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)水平,并统计2组疗效。结果治疗组总有效率93.33%(28/30),对照组总有效率73.33%(22/30),治疗组临床疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分及总分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后CA72-4、TNF-α、IL-6均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后FAACT、KPS评分均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05)。结论健脾益气法联合甲地孕酮片防治结肠癌术后化疗后食欲不振,能够改善食欲不振症状,提高患者生活质量,改善患者炎症反应状态。Objective To study the clinical efficacy of Jianpi Yiqi Prescription combined with megestrol on anorexia in patients with colon cancer(CC)after chemotherapy.Methods Sixty CC patients underwent chemotherapy were randomized 1∶1 to receive oral megestrol(control group)or Jianpi Yiqi Prescription+oral megestrol(treatment group)for 21 days.The traditional Chinese medicine(TCM)syndrome score,Karnofsky Performance Status(KPS)Scale scores,Functional Assessment of Anorexia-Cachexia Therapy(FAACT)scale,tumor marker tumor marker carbohydrate antigen 72-4(CA72-4),serum tumor necrosis factor-α(TNF-α),interleukin 6(IL-6)were included as comparators.The curative effect was assessed.Results The overall effective rate in the treatment group was significantly better than that in the control group(93.33%[28/30]vs 73.33%[22/30],P<0.05).After treatment,the dimension score and total score of TCM syndrome in the both groups were significantly decreased(P<0.05),which were significantly lower in the treatment group than in the control group(P<0.05).CA72-4,TNF-αand IL-6 in the both groups were significantly decreased(P<0.05),which were significantly decreased in the treatment group relevant to the control group(P<0.05).FAACT and KPS scores in the both groups were significantly increased(P<0.05),the increase in the treatment group was more prevalent than the control group(P<0.05).Conclusion For CC patients underwent chemotherapy,Jianpi Yiqi Prescription combined with megestrol has a better effect than megestrol alone,which can improve anorexia,quality of life,and inflammatory response,it has clinical application value.
