摘 要:目的观察荔红汤对葡聚糖硫酸钠(DSS)诱导的溃疡性结肠炎(UC)大鼠的治疗作用及对肠道菌群和炎症反应的影响。方法共选取SD大鼠36只,其中30只采用DSS诱导建立UC模型,剩余6只为对照组。7 d后将造模组大鼠随机分为模型组、柳氮磺吡啶组及荔红汤低、中、高剂量组,每组6只。分别进行灌胃给药,柳氮磺吡啶组予0.35 g/kg的柳氮磺吡啶,荔红汤低、中、高剂量组分别予2、4、6 g/kg的荔红汤提取液,对照组和模型组均予等容积0.9%氯化钠注射液进行灌胃,给药14 d。记录大鼠体质量,观察大鼠粪便性状,检测粪便潜血,评估疾病活动指数(DAI)。取大鼠肠道病变部位进行苏木素-伊红(HE)染色,明确肠道病理情况改变。收集大鼠血清样本,检测炎症因子白细胞介素1β(IL-1β)、IL-18、肿瘤坏死因子α(TNF-α)水平。收集大鼠粪便或肠道内容物,分析肠道菌群组成结构。结果荔红汤中、高剂量组能显著减缓UC大鼠的体质量下降,降低大鼠的DAI评分(P<0.05),显著降低模型大鼠的结肠组织病理学评分(P<0.05),降低UC大鼠血清中IL-1β、IL-18、TNF-α的表达水平(P<0.05),增加大鼠肠道菌群多样性,显著上调大鼠粪便菌群中厚壁菌门的相对丰度(P<0.05),同时下调变形菌门相对丰度(P<0.05),改善模型大鼠的肠道菌群紊乱。结论荔红汤对DSS诱导的UC大鼠疗效良好,可明显改善大鼠结肠炎的病理症状、结肠炎症和肠道菌群失调。Objective To explore the therapeutic effect of Lihong Decoction on dextran sulfate sodium(DSS)-induced ulcerative colitis(UC)rats and its impact on intestinal flora and inflammatory response.Methods A total of 36 SD rats were enrolled.A UC model was established in 30 rats induced by DSS,and the remaining 6 rats were the control group.After 7 days of successful modeling,the rats in the modelling group were randomly divided into model group,sulfasalazine(SASP)group(gavaged by 0.35 g/kg SASP),and Lihong Decoction low,medium and high dose groups(gavaged by 2,4,6 g/kg Lihong Decoction,respectively),with 6 rats in each group;the control group and the model group were given equal volume of 0.9%sodium chloride injection for 14 days.The body weight,fecal traits,and fecal occult blood were recorded,and the disease activity index(DAI)was assessed.The lesion site of the rat intestine was taken for hematoxylin and eosin(H&E)staining to clarify the pathological changes in the intestine.The levels of rat serum inflammatory factors including interleukin-1beta(IL-1β),IL-18,and tumor necrosis factor-alpha(TNF-alpha)were detected.The feces or intestinal contents of rats were collected,and 16SRNA high-throughput sequencing was performed to analyze the composition and structure of intestinal flora of rats in each group.Results After intervention,rats in Lihong Decoction medium and high dose groups presented significantly slowed down the body weight,reduced the DAI score,decreased histopathological score of colon,reduced serum levels of IL-1β,IL-18 and TNF-α,increased the diversity of intestinal flora,and up-regulated relative abundance of Firmicutes in fecal flora,down-regulated relative abundance of Proteobacteria(all P<0.05),and the intestinal flora disorder of model rats was improved.Conclusion Self-made Lihong Decoction has a good therapeutic effect on DSS-induced UC rats,and can significantly improve the pathological symptoms,colitis,and intestinal microbiota imbalance of DSS induced colitis in rats.
