摘 要:目的:基于磁共振SWI和VBM方法探究大脑深髓静脉(DMVs)与脑萎缩及脑小血管病(CSVD)整体负荷的关系。方法:将2021年3月-2023年12月在本院确诊的94例脑小血管病住院患者纳入本研究。应用磁敏感加权成像(SWI)评估DMVs的数目并进行0~3级评分,采用基于体素的形态学测量方法(VBM)通过计算灰质体积分数、白质体积分数及脑脊液体积分数来评估脑萎缩程度,基于常规头颅MR平扫序列(T_(1)WI、T_(2)WI、FLAIR及DWI)评估CSVD总负荷。使用多因素线性回归分析评估DMVs数目及评分与脑萎缩及CSVD总负荷的关系。结果:DMVs数目及评分与CSVD总负荷(β=-0.590,P<0.001;β=0.657,P<0.001)及脑灰质体积分数(β=0.298,P=0.011;β=-0.236,P=0.039)、脑脊液体积分数(β=-0.383,P=0.004;β=0.276,P=0.034)具有显著相关性。结论:DMVs数目及评分与脑萎缩程度及CSVD总负荷密切相关,DMVs或可作为评估CSVD潜在的影像标志物。Objective:The purpose of this study was to investigate the correlation between deep medullary veins(DMVs)and brain atrophy and the total burden of cerebral small vessel disease(CSVD)based on MRI SWI and VBM methods.Methods:A total of 94 patients with CSVD diagnosed in our hospital from March 2021 to December 2023 were enrolled in this study.All subjects were of complete magnetic resonance scan sequence and relatively complete clinical data.The number and 0~3 score of DMVs were assessed using susceptibility-weighed imaging(SWI),voxel-based morphometry(VBM)was used to assess the degree of brain atrophy by calculating gray matter volume fraction,white matter volume fraction,and cerebrospinal fluid volume fraction,and total CSVD burden was assessed by conventional sequence(T_(1)WI,T_(2)WI,FLAIR and DWI).Linear regression analysis was used to evaluate the relationship between the number and score of DMVs and total CSVD burden and brain atrophy.Results:Multivariate linear regression analysis showed that the number and score of DMVs were associated with the total CSVD burden(β=-0.590,P<0.001)and(β=0.657,P<0.001),and gray matter volume fraction(β=0.298,P=0.011)and(β=-0.236,P=0.039),and cerebrospinal fluid volume fraction(β=-0.383,P=0.004)and(β=0.276,P=0.034).Conclusion:The number and score of DMVs are closely correlated with CSVD total burden and brain atrophy,and DMVs may be a potential new imaging marker for CSVD.
