摘　　要：目的 探讨类视黄醇X受体B(RXRB)在子宫内膜癌组织中的表达及临床意义。方法 利用癌症基因组图谱数据库分析RXRB基因在子宫内膜癌组织中的表达。利用免疫组化和实时荧光定量聚合酶链反应(qPCR)技术检测子宫内膜组织中RXRB的表达水平,分析RXRB表达与子宫内膜癌患者临床病理特征的关系及其对预后的影响。结果 在线分析结果发现546例癌组织的RXRB水平[中位数(四分位数)]为28.460(22.671~34.955),低于35例正常组织的43.747(41.999~48.291),差异有统计学意义(P<0.001)。免疫组化和qPCR结果显示:子宫内膜癌组织的RXRB累积光密度值低于正常子宫内膜组织(1.692±0.143 vs.5.263±1.038),RXRB阳性率亦低于正常子宫内膜组织[66.67%(30/45) vs. 86.67%(39/45)],RXRB mRNA水平低于正常子宫内膜组织(0.519±0.249 vs.1.000±0.461),差异有统计学意义(P<0.05)。RXRB表达与淋巴结转移、肿瘤分期和分化程度有关(P<0.05)。单因素分析发现RXRB阳性表达者的中位总生存期未达,优于阴性表达者的18.0个月(HR=9.901,95%CI:2.603~31.762,P<0.001);进一步多因素分析显示RXRB阴性表达是子宫内膜癌患者预后不良的独立危险因子(HR=6.497,95%CI:2.891~14.601,P=0.021)。结论 RXRB低表达与子宫内膜癌的发生、发展有关,往往提示预后不良,该基因有望成为子宫内膜癌的潜在干预靶点。Objective To investigate the expression and clinical significance of retinoid X receptor B(RXRB)in endometrial carcinoma tissues.Methods Expression of RXRB gene in endometrial cancer tissue was analyzed using the Cancer Genome Atlas database.Expression level of RXRB in endometrial tissues were detected byusing immunohistochemistry and real-time fluorescence quantitative polymerase chain reaction(qPCR).Relationship between RXRB expression and clinical pathological characteristics of endometrial cancer patients as well as its impact on prognosis were further investigated.Results Online analysis results showed that the median(quartile)level of RXRB in 546 cancer tissues was 28.460(22.671-34.955),lower than 43.747(41.999-48.291)in 35 normal tissues and the difference was statistically significant(P<0.001).Immunohistochemical and qPCR results showed that the integrated optical density value,positive rate and mRNA level of RXRB were 1.692±0.143,66.67%(30/45)and 0.519±0.249 in endometrial cancer tissues,all lower than 5.263±1.038,86.67%(39/45)and 1.000±0.461 in normal endometrial tissue(P<0.05).RXRB expression was associated with lymph node metastasis,tumor staging,and differentiation degree(P<0.05).Univariate analysis revealed that the median overall survival of individuals with positive RXRB expression was unreached,better than 18.0 months of those with negative expression(HR=9.901,95%CI:2.603-31.762,P<0.001).Further multivariate analysis showed that negative expression of RXRB was an independent risk factor for poor prognosis in endometrial cancer patients(HR=6.497,95%CI:2.891-14.601,P=0.021).Conclusion Low expression of RXRB is associated with the occurrence and development of endometrial carcinoma,which often indicates poor prognosis.This gene is expected to become a potential intervention target for endometrial cancer.
