摘　　要：目的探讨中枢神经细胞瘤(CNC)的流行病学、病理特点,治疗方法及其临床疗效。方法回顾性分析深圳市第二人民医院神经外科2016年2月—2023年2月手术治疗的13例CNC患者的临床特征、影像特征、手术方式、总存活率和无进展存活率,并分析随访结果。结果手术全切11例,次全切2例,其中采用显微镜下经皮质入路10例,全神经内镜手术3例。术后2例出现脑积水,1例保守治疗后自行好转,1例行脑室-腹腔分流术后明显好转;1例出现右侧肢体偏瘫,经康复治疗后改善;1例术后1 h出现急性硬膜外血肿行手术治疗;术后放疗4例,术后随访9~93个月,复发1例。结论最大范围安全切除是CNC主要治疗方式,完全切除后不建议辅助放射治疗,对于肿瘤不完全切除的患者,术后辅助放射治疗可改善患者的预后。Objective To discuss the epidemiology,pathological features,treatment and clinical effectiveness of central neurocytoma(CNC).Methods The clinical characteristics,imaging features,surgical methods,overall survival and progression-free survival of 13 patients with CNC treated surgically in the Department of Neurosurgery,Shenzhen Second People's Hospital were analyzed retrospectively.The follow-up results were analyzed.Results A total of 11 gross total resection and 2 subtotal resection were performed,10 were performed on the subdural cortical microscopic approach and 3 were performed on endoscopic endonasal surgery.After surgery,2 cases of hydrocephalus occurred,1 case improved spontaneously after conservative treatment and 1 case showed significant improvement after ventriculo-peritoneal shunting.After rehabilitation,1 case of right-sided hemiplegia improved.There was 1 case of acute epidural haematoma that occurred 1 hour after surgery and was treated surgically.Postoperative radiotherapy was performed on 4 cases.Postoperative follow-up was from 9 to 93 months,with 1 case of recurrence.Conclusions Maximum safe resection is the primary treatment for CNC.Adjuvant radiotherapy is not recommended after complete resection.For patients with incomplete tumour resection,postoperative adjuvant radiotherapy may improve their prognosis.
