腹腔镜切除成人腹膜后巨大成熟性畸胎瘤一例

摘　　要：腹膜后畸胎瘤主要发生在儿童期,成人发病率低。该病通常无症状,术前明确诊断有难度。成人巨大畸胎瘤恶变率高,建议手术完整切除。报告1例极其罕见的发生在成人起源于腹膜后的巨大成熟性畸胎瘤,其巨大体积和腹膜后间隙解剖位置复杂,临床上对手术技巧、手术方式及切除范围的选择具有挑战性。本例经多学科会诊分析讨论后采取经腹腔镜入路行盆腔腹膜后巨大肿物切除术及宫腔镜下子宫内膜息肉切除术。腹膜后肿物病理最终诊断为成熟性囊性畸胎瘤,患者术后病情平稳,顺利出院。随访4个月未见复发和转移。通过本例以期提高对该病的认识,丰富临床诊治经验,减少误诊和漏诊。Retroperitoneal teratomas mainly occur in childhood with low incidence in adults.The disease is usually asymptomatic and preoperative definitive diagnosis is difficult.Giant teratomas in adults have high rate of malignancy,and complete surgical resection is recommended.We report an extremely rare case of giant mature teratoma retroperitoneal origin in an adult,whose huge size and complex anatomical location in the retroperitoneal space made the selection of surgical technique,surgical approach,and extent of resection clinically challenging.In this case,transperitoneal laparoscopic approach for resection of the huge pelvic retroperitoneal mass and hysteroscopic endometrial polypectomy were adopted after multidisciplinary consultation,analysis and discussion.The final pathological diagnosis of retroperitoneal mass was mature cystic teratoma,and the patient was discharged from hospital with stable condition after surgery.Neither recurrence nor metastasis was seen in the 4-month follow-up.This case aims to improve the understanding of this disease,enrich the clinical diagnosis and treatment experience,and reduce the misdiagnosis and underdiagnosis.
