产后阴道松弛症患者经新型点阵CO_(2)激光联合神经肌肉刺激治疗的效果回顾性分析

摘　　要：目的探究新型点阵CO_(2)激光联合神经肌肉刺激治疗产后阴道松弛症的临床应用效果及其对阴道松弛度、盆底肌功能、性功能的影响。方法回顾性分析产后阴道松弛症患者68例为研究对象,依据治疗方案分为对照组34例(神经肌肉刺激治疗)、研究组34例(新型点阵CO_(2)激光联合神经肌肉刺激治疗)。统计对比两组治疗前后阴道松弛度、盆底肌肌电压(Ⅰ类肌纤维、Ⅱ类肌纤维)、盆底肌肌力(静息、快肌、慢肌)、性功能、阴道黏膜指标及阴道松弛相关指标[血清转化生长因子-β1(TGF-β1)、松弛素(RLX)、雌二醇(E2)]。结果与对照组比较,研究组治疗后阴道松弛度降低(P<0.05);与对照组比较,研究组治疗后Ⅰ类肌纤维、Ⅱ类肌纤维电压升高(P<0.05);与对照组比较,研究组治疗后静息肌力降低,快肌、慢肌肌力升高(P<0.05);与对照组比较,研究组治疗后性功能各维度评分升高(P<0.05);与对照组比较,研究组治疗后阴道黏膜指标各维度评分升高(P<0.05);与对照组比较,研究组治疗后血清TGF-β1、E2水平升高,RLX水平降低(P<0.05)。结论新型点阵CO_(2)激光联合神经肌肉刺激治疗产后阴道松弛症,可有效减轻阴道松弛度,提高盆底肌功能,并可改善性功能、阴道黏膜分泌状态,促进局部组织修复。Objective To explore the clinical effect of a new type of fractional CO_(2) laser combined with neuromuscular stimulation in the treatment of postpartum vaginal relaxation and its impact on vaginal relaxation,pelvic floor muscle function and sexual function.Methods A total of 68 patients with postpartum vaginal relaxation treated were retrospectively analyzed.According to the treatment plan,there were 34 cases in the control group(neuromuscular stimulation therapy)and 34 cases in the study group(new fractional CO_(2) laser combined with neuromuscular stimulation therapy).The vaginal relaxation,pelvic floor muscle voltage(classⅠmuscle fibers,classⅡmuscle fibers),pelvic floor muscle strength(resting,fast and slow muscles),sexual function,vaginal mucosal indexes and vaginal relaxation related indexes[serum transforming growth factor-β1(TGF-β1),relaxant(RLX),estradiol(E2)]were statistically compared between the two groups before and after treatment.Results Compared with the control group,the vaginal relaxation in the study group was decreased after treatment(P<0.05);compared with the control group,the voltage of typeⅠmuscle fiber and typeⅡmuscle fiber in the study group were increased after treatment(P<0.05);compared with the control group,the resting muscle strength in the study group was decreased,while the fast and slow muscle strength were increased after treatment(P<0.05);compared with the control group,the scores of of various dimensions of sexual function in the study group were increased after treatment(P<0.05);compared with the control group,the scores of various dimension of vaginal mucosa index in the study group were increased after treatment(P<0.05);compared with the control group,the levels of serum TGF-β1 and E2 in the study group were increased after treatment,while the level of RLX was decreased(P<0.05).Conclusion The new fractional CO_(2) laser combined with neuromuscular stimulation in the treatment of postpartum vaginal relaxation can effectively reduce vaginal relaxation,improve pelvic
