血清铁蛋白、糖类抗原153及多模态超声对肿瘤最大径≤2 cm乳腺癌的诊断价值

摘  要:目的探讨血清铁蛋白(SF)、糖类抗原153(CA153)与多模态超声在肿瘤最大径≤2 cm乳腺癌诊断中的应用价值。方法选择2020年5月至2023年5月该院收治肿瘤最大径≤2 cm的106例乳腺癌患者为试验组,106例乳腺良性病变患者为对照组。2组患者均接受超微血流显像(SMI)、剪切波弹性成像(SWE)检查及SF、CA153水平检测,分析2组间超声指标及SF、CA153水平的差异,并探讨各指标对乳腺癌的诊断价值。结果试验组患者中SMI恶性病变占比、最大弹性值(E_(max))、平均弹性值(E_(mean))、最小弹性值(E_(min))及SF、CA153水平均高于对照组,差异均有统计学意义(P<0.05)。受试者工作特征曲线分析结果显示,SMI血流分级、E_(max)、E_(mean)、E_(min)、SF、CA153诊断乳腺癌的曲线下面积(AUC)分别为0.740、0.930、0.934、0.793、0.836、0.872,联合诊断的AUC为0.944,高于各指标单独诊断(P<0.05)。结论在乳腺癌患者中,SMI恶性病变占比、E_(max)、E_(mean)、E_(min)及SF、CA153水平均明显升高,且SMI血流分级、E_(max)、E_(mean)、E_(min)、SF、CA153联合对肿瘤最大径≤2 cm乳腺癌具有较高的诊断价值。Objective To investigate the diagnostic value of serum ferritin(SF),carbohydrate antigen 153(CA153)and multimodal ultrasound in the diagnosis of breast cancer with maximum tumor diameter≤2 cm.Methods Totally 106 breast cancer patients with maximum tumor diameter≤2 cm,who were admitted to the hospital from May 2020 to May 2023,were selected as the experimental group.A total of 106 patients with benign breast lesions were assigned to the control group.Both groups of patients underwent super-micro blood flow imaging(SMI),shear wave elastography(SWE)and measurements of SF and CA153 levels.The differences in ultrasound parameters and SF,CA153 levels between the two groups were analyzed,and the diagnostic value of each parameter for breast cancer was further explored.Results In the experimental group,the proportions of malignant lesions on SMI,maximum elasticity value(E_(max)),mean elasticity value(E_(mean)),minimum elasticity value(E_(min))and levels of SF and CA153 were all higher than those in the control group,with statistically significant differences(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)for SMI blood flow grading,E_(max),E_(mean),E_(min),SF and CA153 in diagnosing breast cancer were 0.740,0.930,0.934,0.793,0.836,and 0.872 respectively.The combined AUC was 0.944,which was higher than that of each individual parameter for diagnosis(P<0.05).Conclusion In breast cancer patients,the proportions of malignant lesions on SMI,as well as the values of E_(max),E_(mean),E_(min) and levels of SF and CA153 were significantly elevated.Moreover,the combined use of SMI blood flow grading,E_(max),E_(mean),E_(min),SF and CA153 demonstrated high diagnostic value for breast cancer with maximum tumor diameter≤2 cm.
