摘　　要：目的分析类风湿性关节炎患者(rheumatoid arthritis,RA)外周血淋巴细胞亚群检测意义。方法选取2023年1月~2024年5月浙江省天台县人民医院类风湿性关节炎患者67例(RA组),另选取同期健康体检者86例(对照组)。比较两组淋巴细胞计数(CD45+)、T细胞计数(CD3+)、T抑制细胞计数(CD3+,CD8+)、T辅助细胞计数(CD3+,CD4+)、NK细胞计数(CD16+,CD56+)、B细胞计数(CD19+),将检测指标绘制成ROC曲线,分析其诊断效能。结果RA组淋巴细胞计数(CD45+)、T细胞计数(CD3+)、T抑制细胞计数(CD3+,CD8+)、T辅助细胞计数(CD3+,CD4+)、NK细胞计数(CD16+,CD56+)、B细胞计数(CD19+)均低于对照组,差异均有统计学意义(P<0.01)。ROC曲线AUC、灵敏度、特异度分别为0.871、83.6%和79.1%,均高于淋巴细胞亚群单项检测,差异有统计学意义(P<0.01)。结论RA患者血清淋巴细胞亚群水平均低于健康人群,血清淋巴细胞亚群的联合检测可帮助明确诊断RA。Objective To analyze the significance of detecting peripheral blood lymphocyte subsets in patients with rheumatoid arthritis(RA).Method 67 patients with rheumatoid arthritis from Tiantai County People’s Hospital in Zhejiang Province from January 2023 to May 2024 were selected as the RA group,and 86 healthy individuals who underwent physical examinations during the same period were selected as the control group.Compare the lymphocyte count(CD45+),T cell count(CD3+),T suppressor cell count(CD3+,CD8+),T helper cell count(CD3+,CD4+),NK cell count(CD16+,CD56+),and B cell count(CD19+)between two groups,plot the detection indicators as ROC curves,and analyze their diagnostic efficacy.Result The lymphocyte count(CD45+),T cell count(CD3+),T suppressor cell count(CD3+,CD8+),T helper cell count(CD3+,CD4+),NK cell count(CD16+,CD56+),and B cell count(CD19+)in the RA group were all lower than those in the control group,and the differences were statistically significant(P<0.01).The AUC,sensitivity,and specificity of the ROC curve were 0.871%,83.6%,and 79.1%,respectively,all of which were higher than those of single detection of lymphocyte subsets,and the differences were statistically significant(P<0.01).Conclusion The levels of serum lymphocyte subsets in RA patients are lower than those in healthy individuals,and combined detection of serum lymphocyte subsets can help clarify the diagnosis of RA.
